目的 分析总结6例格林纳达人白内障超声乳化手术特点,为今后对外人道主义医疗服务提供参考.方法 2015年12月5-11日中国海军和平方舟医院船访问格林纳达期间,免费为当地6例(6眼)居民行白内障超声乳化手术,记录其术中及术后出现的特殊情况,分析、总结其特点.结果 6例白内障患者超声乳化术中虹膜松弛明显,术中均出现不同程度瞳孔缩小,虹膜涌动及脱出切口,超声乳化时间及手术时间明显延长;无后囊破裂、无晶状体悬韧带离断、无晶状体皮质及核残留等并发症发生;术后6例患者人工晶状体均位于囊袋,视力明显提高;1例患者前房轻度炎症反应,切口部位虹膜轻度损伤伴脱色素;所有患者无角膜水肿,无瞳孔变形等情况.结论 6例患者白内障超声乳化手术中有虹膜松弛综合征(intraoperative floppy iris syndrome,IFIS)的情况,不排除IFIS与种族有关,这一特点在今后对外人道主义医疗服务中应予以关注.
Abstract
Objective To analyze and summarize the surgical features of 6 cases of cataract phacoemulsification in Grenadian inhabitants,so as to provide reference for future humanitarian medical aid abroad.Methods During the tour by the Chinese Navy Hospital ship "Ark Peace" to Grenada from December 5 to 11,2015,6 cases (6 eyes) received cataract phacoemulsification,free of charge.Special illness conditions encountered during and after surgery were recorded,and the features were analyzed and summarized.Results During phacoemulsification,6 cases had obvious floppy iris and all had pupillary miosis to various extents,with iris undulated and prolapsed toward incision.The time of emulsification and surgery was significantly prolonged.There were no posterior capsule rupture,no breakage of suspensory ligament of lens,no nuclear fragments or cortex lentis fragments and other complications.After surgery,the intraocular lenses of all the patients were in the cystic bag,and visual acuity was improved significantly.There was one patient that had slight inflammatory reaction in anterior chamber,with slight iris damage and depigrnent at incision,but there was no corneal edema and pupilary deformation in all the patients.Conclusions Intraoperative floppy iris syndromes (IFISs) were present in all the cataract phacoemulsification,which might be associated with race,and close attention should be paid to this feature in future foreign humanitarian medical aid.
