摘要
目的 观察针刺联合高压氧治疗对自发性脑出血微创术后患者的康复效果.方法 本研究为随机对照临床研究,选取2019年7月至2022年6月上海中医药大学附属曙光医院神经外科收治的60例自发性脑出血内镜治疗后患者作为研究对象,按随机数字表法分为对照组和试验组,每组30例.对照组患者应用针刺治疗;试验组患者采用针刺联合高压氧治疗,均连续治疗3周.比较2组患者治疗前后的Glasgow预后扩展量表(GOSE)、改良Rankin量表(mRS)、Barthel指数(BI)、美国国立卫生研究院卒中量表(NIHSS)评分和血清脑源性神经营养因子(BDNF)、中枢神经特异性蛋白S100β水平.结果 与治疗前比较,2组患者治疗后的GOSE、BI评分均明显增高,mRS、NIHSS评分明显降低;治疗后试验组患者的GOSE、BI评分明显高于对照组,mRS、NIHSS评分明显低于对照组(P＜0.05).与治疗前比较,2组患者治疗后的血清BDNF水平明显增高、S100β水平明显降低;治疗后试验组患者的血清BDNF水平明显高于对照组,血清S100β水平明显低于对照组(P＜0.05).结论 针刺联合高压氧治疗能更好地促进自发性脑出血微创术后患者的神经功能恢复,提高其生活质量.
Abstract
Objective To observe the rehabilitation effect of acupuncture combined with hyperbaric oxygen therapy on patients with spontaneous cerebral hemorrhage after minimally invasive surgery.Methods This study was a randomized controlled clinical study.A total of sixty patients with spontaneous cerebral hemorrhage treated with endoscopy in the Department of Neurosurgery of Shuguang Hospital affiliated to Shanghai University of Traditional Chinese Medicine from July 2019 to June 2022 were selected as the research subjects,and were divided into control group and experimental group,according to the random number table method,with 30 patients in each group.The control group was treated with acupuncture,and the experimental group was treated with acupuncture combined with hyperbaric oxygen therapy.Both groups were treated continuously for three weeks.The Glasgow outcome scale extended(GOSE),modified Rankin scale(mRS),Barthel index(BI),National Institutes of Health stroke scale(NIHSS)scores,serum brain-derived neurotrophic factor(BDNF),and central nervous system specific protein S100β levels were compared between the two groups before and after treatment.Results After treatment,the scores of GOSE and BI in both groups were significantly increased,while the scores of mRS and NIHSS were significantly decreased in both groups.After treatment,the scores of GOSE and BI in the experimental group were significantly higher than those in the control group,while the scores of mRS and NIHSS in the experimental group were significantly lower than those in the control group(P＜0.05).After treatment,the serum levels of BDNF in both groups were significantly increased,while the serum level of S100β in both groups were significantly decreased.After treatment,the serum level of BDNF in the experimental group was significantly higher than that in the control group,and the serum level of S100β in the experimental group was significantly lower than that in the control group(P＜0.05).Conclusion Acupuncture combined with hyperbaric oxygen therapy can better promote the recovery of neurological function and improve quality of life and neurological function of the patients with spontaneous cerebral hemorrhage after minimally invasive surgery.
