目的 观察高压氧(HBO)联合前列地尔在先天性心脏病(CHD)伴重度肺动脉高压(PH)患者围手术期的应用效果.方法 选取2021年1月至2023年1月临沂市中心医院心脏大血管外科收治的118例CHD合并重度PH患者,采用双盲、抽签的方法将患者纳入单一组(58例)或联合组(60例).2组患者均接受常规手术治疗,单一组从术前7 d至术后7 d接受前列地尔治疗,联合组按单一组方案接受前列地尔治疗并从术前15 d开始接受HBO治疗.比较2组患者入院时(T0)、术前接受前列地尔治疗7 d后(T1)和术后第7天(T2)各个时间点的血流动力学指标,包括平均肺动脉压(MPAP)、平均体循环压(MSAP)、体循环阻力(SVR)、肺循环阻力(PVR)、肺动脉压/主动脉压(Pp/Ps)及动脉血氧饱和度(SaO2);比较2组患者主动脉阻闭时间、体外循环时间、呼吸机带管时间、重症监护病房(ICU)住院时间.结果 2组患者TO时的血流动力学指标差异无统计学意义(P>0.05);联合组在T1、T2时的MPAP、MSAP、SVR、PVR、Pp/Ps低于单一组,SaO2高于单一组,差异均有统计学意义(P<0.05).2组患者体外循环时间、主动脉阻闭时间差异无统计学意义(P>0.05);联合组呼吸机带管时间和ICU住院时间短于单一组,差异均有统计学意义(P<0.05).结论 与单独使用前列地尔相比,CHD合并重症PH患者围手术期使用HBO联合前列地尔可显著降低MPAP、提高SaO2,有效缩短呼吸机带管时间和ICU住院时间.
Application of hyperbaric oxygen combined with alprostadil in the treatment of congenital heart disease with severe pulmonary hypertension in perioperative period
Objective To observe the effect of hyperbaric oxygen(HBO)combined with alprostadil in the treatment of congenital heart disease(CHD)with severe pulmonary hypertension(PH)during the perioperative period.Methods A total of 118 patients with CHD combined with severe PH admitted to Department of Cardiovascular Surgery of Linyi Central Hospital from January 2021 to January 2023 were selected.They were divided into single treatment group(58 cases)and combined treatment group(60 cases)by double-blind and lottery method.Patients in both groups received conventional surgical treatment;the single treatment group received alprostadil from seven days before surgery to seven days after surgery,and the combined treatment group received alprostadil according to the regimen of the single treatment group and HBO therapy 15 days before surgery.The hemodynamic indexes of the two groups were compared at admission(T0),seven days after preoperative treatment with alprostadil(T1),and seven days after surgery(T2):mean pulmonary artery pressure(MPAP),mean systemic arterial pressure(MSAP),systemic vascular resistance(SVR),pulmonary vascular resistance(PVR),pulmonary arterial pressure/aortic pressure(Pp/Ps),and arterial oxygen saturation(SaO2).Aortic occlusion time,cardiopulmonary bypass time,ventilator time,and ICU stay were compared between the two groups.Results There was no significant difference in hemodynamic indexes at T0 between the two groups(P>0.05).The indicators of MPAP,MSAP,SVR,PVR,and Pp/Ps of the combined treatment group were lower than those of the single treatment group at T1 and T2,but SaO2 of the combination treatment group was higher than that of the single treatment group,with statistically significant differences(P<0.05).There was no significant difference in cardiopulmonary bypass time and aortic occlusion time between two groups(P>0.05).The duration of ventilator intubation and ICU stay in the combined treatment group were shorter than those in the single treatment group,and the differences were statistically significant(P<0.05).Conclusion Compared with using alprostadil alone,HBO combined with alprostadil in CHD patients with severe PH during perioperative period can significantly reduce MPAP and increase SaO2,thus effectively shortening the time of ventilator use and ICU stay.
