Analysis of the disease characteristics of newly diagnosed malignant tumors in 1 638 elderly patients covered by a medical care system
Objective To clarify the occurrence characteristics,disease spectrum,and changing trends of 1638 elderly patients covered by a medical care system with newly diagnosed malignant tumors.Methods Based on histopathological reports,the medical records were retrospectively collected on 1 638 elderly patients aged 60 and above with newly diagnosed malignant tumors admitted to the Cadres'Diagnosis and Treatment Department of the Sixth Medical Center of PLA General Hospital from January 1993 to December 2022.The patients,their family members,treating physicians,and healthcare physicians were interviewed through questionnaires by uniformly trained investigators to collect demographic information such as age,gender,and occupation,as well as clinical information including the year of confirmed diagnosis,diagnosis,and tumor staging of malignant tumors.Malignant tumors were coded using the International Classification of Diseases(ICD)-10.Taking 5 years as a period,the overall occurrence ranking and changing trend of malignant tumors,as well as the differences in the incidence among different genders,ages,and years were analyzed.Results Among the 1 638 elderly patients with newly diagnosed malignant tumors,1 460 cases were male and 178 were female.The case number of patients with three and two types of malignant tumors simultaneously were 13(3 cases were female)and 74(8 cases were female),respectively.The number of newly-diagnosed cases of malignant tumors showed a continuous upward trend from 1993 to 2017,while the number from 2018 to 2022 decreased significantly compared with that before,with a statistical significant difference(P<0.05).The top 5 newly diagnosed malignant tumors in those elderly patients were lung cancer(29.22%),prostate cancer(12.44%),colorectal cancer(11.30%),gastric cancer(7.48%),and liver cancer(6.34%).The incidence of lung cancer had been increasing year by year since the age of 60,and was the highest in the age group of 70-79.Prostate cancer was most prevalent in the age group of 80-89.Colorectal cancer mainly occurred in the people over 70 years old,with the highest incidence in the age group of 80-89.The occurrence ages of gastric cancer and liver cancer were relative early,and there were relative high incidences in all age groups.Bladder cancer mainly occurred in people over 70 years old.Pancreatic cancer accounted for 53.85%of the population aged 80-89 years.Conclusion The number of newly-diagnosed malignant tumors in the elderly population under a medical care system showed a trend of rapid growth,slow growth,and then gradual decline.The top five malignant tumors with the highest incidence were lung cancer,prostate cancer,colorectal cancer,gastric cancer,and liver cancer,which were different from the cancer distribution reported in Beijing.Early prevention and control of tumors in the digestive,respiratory,and urogenital systems of this population should be emphasized.
