基于18F-FDG PET代谢参数预测软组织肉瘤组织学分级的价值研究

目的 评估18F-FDG PET代谢参数在预测软组织肉瘤(STS)组织学分级中的应用价值.方法 回顾性收集2012年12月至2021年12月治疗前于大连医科大学附属第一医院行18F-FDG PET/CT显像并经病理学证实为STS的患者51例(男26例、女25例,年龄范围:32~84岁).测量FDG代谢参数SUVmax、肿瘤代谢体积(MTV)、病灶糖酵解总量(TLG)和肿瘤内FDG摄取异质性(IFH).采用Kruskal-Wallis秩和检验分析不同组间各代谢参数的差异,采用Spearman秩相关分析各代谢参数与STS组织学级别的相关性,采用logistic回归分析筛选并构建高级别STS的预测模型,绘制ROC曲线并采用Delong检验分析不同AUC间的差异.结果 SUVmax、MTV、TLG和IFH在法国癌症中心肉瘤组织联合会(FNCLCC)Ⅰ(n=8)、Ⅲ(n=10)及Ⅲ级(n=33)STS组间差异均有统计学意义(H值:16.24、10.52、19.29和16.99,均P<0.05),各代谢参数均与STS的组织学级别呈正相关(rs值:0.58、0.45、0.52 和 0.62,均P<0.05).多因素 logistic 回归分析示 SUVmax[比值比(OR)=1.27,95%CI:1.06~1.51,P=0.009]和 IFH(OR=6.83,95%CI:1.44~32.27,P=0.015)是高级别 STS 的独立预测因子,二者相结合所构建的预测模型对高级别STS的诊断AUC为0.93,灵敏度和特异性分别为93.9%(31/33)和 16/18,预测模型与 SUVmax、MTV、TLG 和 IFH 的 AUC(0.81、0.78、0.86 和 0.85)相比,差异均具有统计学意义(z值:2.69、2.53、1.94和1.97,均P<0.05).结论 18F-FDG PET代谢参数SUVmax、MTV、TLG和IFH是STS组织学分级有价值的预测因素;与单独使用各代谢参数相比,联合应用SUVmax和IFH可能是一种预测STS组织学分级更有意义的方法.
Application of 18F-FDG PET metabolic parameters in evaluating histopathologic grading of soft tis-sue sarcoma
Objective To evaluate the value of 18F-FDG PET metabolic parameters in predicting histopathological grade of soft tissue sarcoma(STS).Methods From December 2012 to December 2021,51 patients(26 males,25 females,age range:32-84 years)who underwent 18F-FDG PET/CT imaging be-fore treatment and confirmed STS pathologically in the First Affiliated Hospital of Dalian Medical University were retrospectively collected.18F-FDG PET metabolic parameters SUVmax,metabolic tumor volume(MTV),total lesion glycolysis(TLG)and intertumoral FDG uptake heterogeneity(IFH)were measured.Kruskal-Wallis rank sum test was used to analyze the differences in metabolic parameters among different groups and Spearman rank correlation analysis was used to analyze the correlation of each metabolic parame-ter and histological grade.Logistic regression was used to screen and construct the prediction model for high-grade STS.ROC curve was plotted and Delong test was used to analyze the differences among AUCs.Results The metabolic parameters SUVmax,MTV,TLG and IFH were significantly different among French Federation of Cancer Centers Sarcoma Group(FNCLCC)Ⅰ(n=8),Ⅱ(n=10)and Ⅲ(n=33)grade groups(H values:16.24,10.52,19.29 and 16.99,all P<0.05),and each metabolic parameter was positively correlated with histo-logical grade(rs values:0.58,0.45,0.52,and 0.62,all P<0.05).Multivariate logistic regression analysis showed that SUVmax(odds ratio(0R)=1.27,95%CI:1.06-1.51,P=0.009)and IFH(OR=6.83,95%CI:1.44-32.27,P=0.015)were independent risk indicators for high-grade STS.The prediction model constructed by combining SUVmax and IFH had better diagnostic efficacy for differentiating high-grade STS with the AUC of 0.93,and the sensitivity of 93.9%(31/33)and the specificity of 16/18,respectively.The AUC of predic-tion model was significant different from SUVmax,MTV,TLG and IFH(AUCs:0.81,0.78,0.86 and 0.85;z values:2.69,2.53,1.94 and 1.97,all P<0.05).Conclusions The metabolic parameters SUVmax,MTV,TLG and IFH are valuable predictors for histological grade of STS.The combination of SUVmax and IFH may be a more meaningful method than using each of the above metabolic parameters alone.
