摘要
目的对比研究低位肛瘘瘘管切开术中FOCUS超声刀与常规手术的应用。方法采集2011年3月至2013年2月首都医科大学附属北京朝阳医院京西院区收治的行低位肛瘘瘘管切开术患者136例，随机分为观察组（ FOCUS超声刀组）69例和对照组（常规手术）67例，比较两组患者的手术时间、术中失血量、术后并发症。结果观察组和对照组的手术时间分别为（37.2±6.3） min和（49.6±8.9） min，差异有统计学意义（t＝18.19，P＝0.001），术中失血量分别为（25.3±5.7）ml和（39.6±6.2）ml，差异有统计学意义（ t＝12.57，P＝0.001）；二组术后出血、延迟愈合、肛门失禁等并发症发生情况比较，差异无统计学意义（χ2＝0.78，P＝0.53）。结论应用超声刀行痔切除术可以明显地减少手术的出血量、缩短手术时间，不增加手术并发症的发生率。
Abstract
Objective To compare the efficacy of FOCUS ultrasonic knife with common operation in the treatment of low fistula canal dissection .Methods A total of 136 cases from Beijing Chaoyang Hospital between March 2011 and February 2012 , were divided into two groups .The FOCUS ultrasonic knife was used in the observation group ( n=69 ) and the common operation was used in the control group ( n=67 ) .The operation time , intraoperative blooding volume ,complications and other indicators after operation were statistically analyzed .Results The operation time of observation group and control group were as follows :observation group(37.2 ±6.3)min vs. control group(49.6 ±8.9)min.The difference in the operation time was statistically significant (t =18.19,P =0.001 ) .The intraoperative blooding volume of observation group and control group were ( 25.3 ±5.7 ) ml and (39.6 ±6.2) ml.The differences in the intraoperative blooding volume was statistically significant ( t =12.57, P=0.001).No significant differences was observed in postoperative bleeding ,delayed union and fecal incontinence (χ2 =0.78 , P =0.53 ) .Conclusion Compared with the common operation , FOCUS ultrasonic knife can reduce intraoperative blooding volume and operation time with less incidence rate of complications .
