摘要
随着人们生活水平的提高，糖尿病的患病率也随之增加，医疗负担的加重，合理规划和改善糖尿病的管理逐渐成为社会各界关注的重点。为了满足糖尿病患者的医疗需求，提供优质的综合管理服务，一系列政策倡导糖尿病管理应逐步向社区延伸。通过梳理国内外经典的糖尿病社区管理模式的相关文献，分析当前社区管理的现状及不足，为制定科学的、有效的糖尿病社区管理模式提供理论依据。
Abstract
With the improvement of people's living standards, the rising prevalence of diabetes, and the increasing medical burden, planning and optimizing diabetes management have gradually become the focus of society. In order to meet the medical needs of diabetic patients and provide high-quality comprehensive management services, a series of policies advocate that diabetes management should be extended to the community. By sorting out the relevant literature on classic diabetes community management models at home and abroad, this article analyzes the current status and deficiencies of community management, and provides a theoretical basis for formulating scientific and effective diabetes community management model.
