This article reported a case of a 58-year-old patient who was admitted for a progressively enlarged right scrotum over the past 20 years and being painful for 3 days.Preoperative MR examination showed a solid mass above the right inguinal region of the scrotum,suggesting a malignant possibility.Under general anesthesia,a laparoscopic high ligation and disconnection of the spermatic cord above the peritoneum+tension-free repair of the right inguinal hernia by laparoscopy+excision of the scrotal mass+right testicular appendage and epididymis resection(scrotal incision)+right iliac vessel lymph node resection were performed.Postoperative pathological diagnosis was scrotal myxofibrosarcoma.Three months of follow-up revealed no recurrence on pelvic MR.Scrotal myxofibrosarcoma is a rare scrotal disease,and there is currently no unified standard for the treatment.However,local extensive excision is usually adopted,and postoperative radiation therapy may be considered for tumors with positive margins or recurrence.Due to the low malignancy of myxofibrosarcoma,the overall prognosis is relatively good.
