鼾症患儿常规手术与日间手术后睡眠和康复质量的比较

摘要
目的 比较鼾症患儿常规手术与日间手术后睡眠和康复的质量.方法 本研究为前瞻性研究.选择2023年3月至6月广州医科大学附属广州市妇女儿童医疗中心行腺样体和/或扁桃体切除术鼾症患儿70例,年龄4～6岁,ASA分级Ⅰ或Ⅱ级,分为常规手术组(R组)和日间手术组(D组),每组35例.采用荣耀手环5监测睡眠状况,分别于术前晚、术后第1晚和术后第2晚记录深睡眠比例、浅睡眠比例、快动眼睡眠比例、睡眠连续性评分、最高心率和最低心率.分别于术前1 d、术后24、48 h和28 d时采用15项生活质量评分量表(QoR-15)评估康复情况.结果 与R组比较,D组术前晚最高心率降低,术后第2晚浅睡眠比例升高,快动眼睡眠比例降低,术后24和48 h可以获得医护帮助条目评分、感觉舒适条目评分、感觉健康条目评分和QoR-15总分均降低,术后48 h时感觉精力充沛条目评分和拥有好睡眠条目评分降低(P＜0.05).结论 相对于常规手术而言,鼾症患儿日间手术后早期睡眠和康复质量较差.
Abstract
Objective To compare the quality of sleep and rehabilitation after routine surgery ver-sus day surgery in pediatric patients with snoring.Methods This was a prospective study.Seventy pediatric patients with snoring,aged 4-6 yr,of American Society of Anesthesiologists Physical Status classification Ⅰ orⅡ,undergoing adenoidectomy and/or tonsillectomy in the Guangzhou Women and Children's Medical Center from March to June 2023,were divided into routine surgery group(group R)and day surgery group(group D),with 35 cases in each group.A sleep-detecting bracelet was used to monitor the sleep status.The propor-tion of deep sleep,light sleep and rapid eye movement,sleep continuity score and the maximum and minimum heart rate were recorded on the night before surgery,the first and second night after surgery.Rehabilitation was assessed using Quality of Recovery-15 at 1 day before surgery and 24 and 48 h and 28 days after surgery.Results Compared with group R,the maximum heart rate at the night before surgery was significantly de-creased,the proportion of light sleep on the second night after surgery was increased,the proportion of rapid eye movement sleep on the second night after surgery was decreased,the scores for items on"getting support from hospital doctors and nurses","feeling comfortable and in control",and"having a feeling of general well-being"and the total score of Quality of Recovery-15 were significantly decreased at 24 and 48 h after op-eration,and the score for items on"feeling rested"and"have had a good sleep"at 48 h after surgery was de-creased in group D(P＜0.05).Conclusions Pediatric patients with snoring have poor quality of early sleep and rehabilitation after day surgery compared with routine surgery.
