摘要

目的 评价核因子E2相关因子2(Nrf2)/血红素加氧酶-l(HO-1)信号通路在电针预处理改善老龄大鼠术后认知功能中的作用.方法 清洁级健康雄性SD大鼠80只,24月龄,体质量550~600 g,采用随机数字表法分为4组(n=20):对照组(C组)、POCD组、电针+POCD组(EA组)和Nrf2抑制剂ML385+电针+POCD组(I组).采用3%七氟烷麻醉下剖腹探查术的方法制备大鼠POCD模型.I组大鼠于术前30 min腹腔注射Nrf2抑制剂ML385 15 mg/kg,其余组给予等量生理盐水.EA组和I组大鼠于模型制备前5 d每天早晨8:00对足三里、合谷、内关穴进行5 d的电针刺激(疏密波,电流强度1 mA,频率2/15 Hz,30 min).术后3 d时采用旷场实验检测大鼠自发运动能力,术后3~7 d旷场实验结束后采用Morris水迷宫实验评估大鼠认知功能.水迷宫实验结束,麻醉大鼠处死后取海马组织,采用流式细胞术测定活性氧(ROS)水平,Western blot法检测Nrf2和HO-1的表达,透射电子显微镜下观察海马神经元线粒体超微结构.结果 与C组比较,POCD组大鼠逃避潜伏期延长,穿越原平台位置次数减少,海马ROS水平升高,Nrf2和HO-1表达下调(P<0.05),海马CA1区神经元线粒体肿胀,线粒体嵴结构断裂;与POCD组比较,EA组大鼠逃避潜伏期缩短,穿越原平台位置次数增加,海马ROS水平下降,Nrf2和HO-1表达上调(P<0.05),海马CA1区神经元线粒体肿胀及嵴结构断裂情况改善;与EA组比较,I组大鼠逃避潜伏期延长,穿越原平台位置次数减少,海马ROS水平升高,Nrf2和HO-1表达下调(P<0.05),海马CA1区神经元线粒体肿胀及嵴结构断裂情况加重.结论 海马Nrf2/HO-1信号通路激活参与了电针预处理改善老龄大鼠术后认知功能的过程.

Abstract

Objective To evaluate the role of nuclear factor erythroid 2(Nrf2)/heme oxygenase-1(HO-1)signaling pathway in the improvement in postoperative cognitive function by electroacupuncture pre-conditioning in aged rats.Methods Eighty clean-grade healthy male Sprague-Dawley rats,aged 24 months,weighing 550-600 g,were divided into 4 groups(n=20 each)using a random number table meth-od:control group(C group),POCD group,electroacupuncture+POCD group(EA group),and Nrf2 inhib-itor ML385+electroacupuncture+POCD group(I group).A rat POCD model was prepared by performing ex-ploratory laparotomy under 3%sevoflurane anesthesia.Nrf2 inhibitor ML385 15 mg/kg was intraperitoneally injected at 30 min before surgery in group Ⅰ,while the equal volume of normal saline was given instead in the remaining groups.Electroacupuncture stimulation of the Zusanli,Hegu and Neiguan acupoints(dis-perse-dense waves,current intensity 1 mA,frequency 2/15 Hz,30 min)was performed at 8:00 a.m.every day for 5 days prior to model preparation in EA group and I group.An open field test was conducted at 3 days after operation to measure the autonomous motor function,and Morris water maze test was conducted af-ter the end of the open field test at 3-7 days after operation to evaluate the cognitive function.After the Mor-ris water maze test,the rats were sacrificed under anesthesia and hippocampal tissues were taken for deter-mination of the level of reactive oxygen species(ROS,by flow cytometry)and expression of Nrf2 and HO-1(by Western blot)and for examination of the mitochondrial ultrastructure of hippocampal neurons(with a transmission electron microscope).Results Compared with group C,the escape latency was significantly prolonged,the number of crossing the original platform quadrant was decreased,the hippocampal ROS lev-els were increased,the expression of Nrf2 and HO-1 was down-regulated(P<0.05),and mitochondria were swollen and mitochondrial cristae were broken in the hippocampal CA1 neurons in group POCD.Compared with group POCD,the escape latency was significantly shortened,the number of crossing the original plat-form quadrant was increased,the hippocampal ROS levels were decreased,the expression of Nrf2 and HO-1 was up-regulated(P<0.05),and the swelling of mitochondria and rupture of cristae structure were improved in the hippocampal CA1 neurons in group EA.Compared with group EA,the escape latency was significant-ly prolonged,the number of crossing the original platform quadrant was decreased,the hippocampal ROS levels were increased,the expression of Nrf2 and HO-1 was down-regulated(P<0.05),and the swelling of mitochondria and rupture of cristae structure were aggravated in the hippocampal CA1 neurons in group Ⅰ.Conclusions Activation of the hippocampal Nrf2/HO-1 signaling pathway is involved in improvement in postoperative cognitive function by electroacupuncture preconditioning in aged rats.
