病案延迟归档的原因及改进

摘要
目的 探讨提高普通病案3日归档率,死亡病案7日归档率的对策.方法 采用卡方检验对2014年7月-12月和2015年3月-8月22841份出院病案归档率进行统计和分析.结果 干预前10887份出院病案中归档5045份,归档率为46.3%,采取措施后,分析2015年3月-8月11954份出院病案,归档8630份,归档率为72.2%,提高了25.9%.结论 医院各级部门应重视病案归档工作,病案管理委员会要发挥其相应的职能,建立有效的病案管理制度和奖惩制度是提高病案按期归档率的有效措施.
Abstract
Objective To investigate the countermeasures of improving filing efficiency of ordinary medical record in 3 days and death medical record in 7 days.Methods X2 test was used to conduct statistical analysis on the filing rate of 22841 discharged medical records from July to December in 2014 and March to August in 2015. Results In the first 10887 cases,5045 cases were discharged from hospital discharge records,the filing rate was 46.3%, and the measures were taken to extract the medical records of 11954 cases from to 2015,8630 copies were filed, and the filing rate increased to 72.2%, which increased by 25.9%.Conclusions The hospital departments at all levels should pay attention to the work of medical record archiving, and the Medical Record Committee should be responsible for relevant functions. To establish effective medical records management rules as well as reward and punishment system are effective measures to improve the archiving rate of medical records.
