A Pass-through Analysis of Hospitalization Costs and Factors Influencing HIV/AIDS Surgery Patients
Objectives This study aims to understand the composition and influencing factors of hospitalization cost of HIV/AIDS surgery patients,so as to provide a reference for reasonable control of hospitalization cost and optimization of medical service price system.Methods The front page information of 316 HIV/AIDS surgery patients in Nantong AIDS designated hospitals from January 1,2017 to December 31,2021 were collected to conduct a descriptive analysis of the composition of the patients'hospitalization costs and to explore the factors influencing their hospitalization costs using through-path analysis.Results There were 236(74.7%)HIV-infected patients and 80(25.3%)surgical patients with AIDS.Median length of stay was 10 and 21 days.The median hospitalization expenses were 16,690.7 yuan and 36,251.2 yuan.The average cost per time for HIV/AIDS surgery patients in 5 years was 28,383.8 yuan,with a compound annual growth rate of 6.1%,and the length of stay and cost for AIDS patients were both higher than those for HIV-infected patients.The difference was statistically significant.The main components of hospitalization expenses were consumables(32.4%),drug expenses(27%),and diagnostic expenses(18.6%).The results of the path analysis showed that the number of days in hospital was the main influencing factor of hospitalization cost.Gender,age,and disease type not only had direct effects on hospitalization cost,but also indirectly influenced hospitalization cost through the number of days in hospital.Different disease stages indirectly influenced hospitalization cost through the number of days in hospital.Conclusions The hospitalization cost of HIV/AIDS surgery patients is increasing,and the cost of consumables and drugs account for a large proportion of the cost.The primary factor affecting hospitalization costs is the length of stay.The length of hospitalization and costs for AIDS patients are higher than those for HIV-infected patients,and the difference is statistically significant.Medical institutions should strengthen the management of controllable factors,focus on shortening the length of hospitalization,and effectively reduce the financial burden on patients.
