A Study on Reasons of 1194 Patients for Discharge Against Medical Advice
Objectives To analyze the reasons and related characteristics of discharge against medical advice(AMA),so as to provide a basis for reducing the occurrence of adverse consequences of discharge and optimizing hospital management.Methods Based on the front page data of 1194 patients discharged from a secondary hospital in Beijing from October 1st,2021 to September 30th,2022,the reasons for their discharge against medical advice(AMA).Results Among 24,093 discharged patients,4.96%were discharged without medical advice.Multivariate logistic regression analysis showed that non-city(OR=1.318),male(OR=1.162),payment methods were other methods(OR=1.438),admission routes were emergency(OR=3.982),transfer(OR=1.278),hospitalization days were less than or equal to 7 days(OR=2.819),no surgery(OR=0.312),and the total cost was less than 9550.80 yuan(OR=1.352)was a risk factor for non-physician discharge(P<0.05).Patients believed that improvement or improvement of symptoms was the main reason for AMA,and 694(58.12%)were discharged.Conclusions The demographic characteristics of patients,admission,length of hospitalization,and diseases had an important impact on discharge against medical advice.improving the graded diagnosis and treatment system,continuously improving the quality of medical care and strengthening the communication between doctors and patients.It could reduce the adverse consequences and effects of AMA.
