Analysis of PDCA Method on the Improvement of DRGs Indicator in a Hospital
Objectives Through PDCA quality control management mode,the data quality of the front page of inpatient medical records was refined,aiming to strengthen and improve the DRGs index of a hospital,and provide data support for improving the new efficiency of high-quality development of hospitals.Methods From January 1st,2020 to December 31st,2020,the front page of 20 751 hospitalized medical records before PDCA quality control management was selected as the control group,and from January 1st,2021 to December 31st,2022,after PDCA quality control management,46 901 hospitalized medical records were selected as the study group.Through the inpatient medical service performance evaluation platform in Beijing,we extracted the representative cardiology and urology departments of the whole hospital and the service aviation medicine,as well as the orthopedics and respiratory departments with the largest number of beds,and extracted the DRGs related index data of the city's tertiary general hospitals.Including enrollment rate,DRGs group number,CMI,time consumption index,cost consumption index,low risk mortality.According to the DRGs indicators before implementation,7 target values are set and compared.Results After the implementation of PDCA quality control management,the enrollment rate of a hospital in 2021 and 2022 reached 99.8％,and the CMI reached 1.05.In 2021,compared with 2020,the increase rate of enrolled cases reached 20.95％,the decrease rate of low-risk mortality reached 70％,the decrease rate of cost consumption index reached 4.67％,and the decrease rate of time consumption index reached 2.65％.In 2022,compared with 2021,the decrease rate of enrolled cases reached 12.89％,the decrease rate of low-risk mortality reached 33.3％,the decrease rate of cost consumption index reached 9.8％,and the decrease rate of time consumption index reached 5.45％.By 2022,the number of DRGs groups was 212 fewer than that of the city's tertiary general hospitals,and the time consumption index was still higher than the city's average.More than 4 indexes in cardiology,urology,orthopedics and respiratory departments reached the target value.Conclusions With the application of PDCA quality control management could continuously improve the DRGs index of a hospital.It was beneficial to strengthen the medical quality management of the hospital,shorten the hospitalization time,control the medical cost and improve the medical technology level,and more effectively enhance the medical service capacity of the hospital.
