Effect of PDCA Management on Delayed Filing of Medical Records in Different Departments
Objectives This study aims to explore the current situation of delayed medical record filing in different departments and analyze the effect of Deming cycle(Plan-Do-Check-Act cycle,PDCA)in managing late filing of medical records.Methods All discharged medical records from all departments of the hospital from January 1,2021 to Februaty 28,2023 were retrospectively selected and the filing of medical records was analyzed.The delay rate of discharge records for more than 3 days was observed,the causes were analyzed,and scientific and reasonable improvement strategies were proposed for clinical process management.Results In 2021,a total of 184,599 patients were discharged from a certain hospital,and the number of late return medical records was 1340,with a late return rate of 0.73%.Due to the epidemic,only 7 months of statistics were collected in 2022.A total of 115 436 patients were discharged from the hospital,1456 medical records were late,and the late return rate was 1.26%.Only 2 months of statistics were collected in 2023,after which the hospital gradually applied the PDCA management method.A total of 31,031 people were discharged,108 medical records were late,and the late return rate was 0.35%.Compared with before the application of PDCA management method,the late return rate of medical records was lower after the use of PDCA management,and the difference was statistically significant.Conclusions Late filing was more common in the burn department,urology department Ⅱ,respiratory department I,pancreatic surgery,liver-laparoscopic surgery,and general surgery/special surgery.The PDCA management method can effectively improve the timely filing rate of medical records and is suitable for clinical promotion.
