Analysis on Influencing Factors of Hospitalization Expenses in Elderly Patients with Type 2 Diabetes Based on Quantile Regression
Objectives To explore the influencing factors of hospitalization expenses in elderly patients with type 2 diabetes,and provide reference for scientific and reasonable control of medical expenses.Methods The data of 1,856 discharged patients aged above 60 years with type 2 diabetes were collected from a Three A and Tertiary Hospital in Beijing between January 1st,2018,and December 31st,2022.The demographic and clinical information including gender,age,nationality,marital status,occupational status,patient source,transfer to another department status,surgical intervention status,presence of complications,length of hospital stay as well as the payment method and total costs associated with hospitalization including drug and consumable expenses were extracted.After adjusting for cost discounts,a nonparametric test was employed to conduct univariate analysis.Multivariate analysis was performed using the quantile regression model at the 10th,50th,and 90th percentile levels of hospitalization costs.Results The median hospitalization expenses of the patients in the study was 10147.91 yuan.The univariate analysis revealed that age,occupational status,transfer to another department status,surgical intervention status,occurrence of complications,length of hospitalization stay,payment method,drug proportion and consumption proportion were significant determinants of hospitalization expenses(P<0.05).Quantile regression analysis revealed significant associations between age and hospitalization expenses at both the lower and upper quantiles(P<0.05).Marital status and payment method were found to impact hospitalization expenses at the lower quantiles(P<0.05),while patient source had an effect on hospitalization expenses at the higher quantiles(P<0.05).Additionally,variables such as length of stay,presence of complications,surgical procedures,drug proportion,and consumption proportion exhibited significant impacts on hospitalization expenses across all quantiles(P<0.05).Conclusions Quantile regression analysis suggested that the number of days in hospital,surgery during hospitalization,complications,and application of consumables were the main factors affecting the hospitalization costs of elderly patients with type 2 diabetes.Shortening the number of days in hospital,strengthening patient management,and promoting the rational use of drugs and consumables were important ways to control the economic burden of elderly patients with type 2 diabetes.
