TOPSIS Analysis on the Medical Quality in a Tertiary Comprehensive Hospital Based on DRGs
Objectives To conduct an in-depth evaluation and analysis on the medical quality of tertiary comprehensive hospitals using the TOPSIS method, and provide decision-making basis for the high-quality development and refined management of hospitals. Methods The DRG related information of patients discharged from various departments of the hospital in 2023 was obtained through the Shanxi Province DRG Comprehensive Service Platform, and the TOPSIS method was applied to comprehensively evaluate the inpatient medical performance of the departments. Results The main diagnostic classification(MDC)group of the diseases admitted to the hospital was 21,and the DRG group was 430,accounting for 62.05%of all groups. The cost consumption index was 1.02,slightly lower than the city average level, while the time consumption index was 1.15,slightly higher than the city average level. The CMI was 1.11,and the overall technical difficulty was slightly higher than the average level in this city. The ICU cost consumption index was the highest(4.23),the radiation therapy department had the highest time consumption index(2.13),and the efficiency of inpatient services was relatively low.The widest range of disease diagnosis and treatment was in the liver, gallbladder, pancreas, and stomach surgery department with 171 groups, while the narrowest range was in the ICU with 7 groups. The weighted TOPSIS method comprehensive evaluation results showed that GCP,gastroenterology, and traditional Chinese medicine ranked in the top three;ICU,thoracic surgery, and colorectal surgery ranked last three. Conclusions The TOPSIS analysis based on DRG had demonstrated its unique advantages and value in the evaluation of medical quality in a tertiary comprehensive hospital. Not only provide a reasonable and feasible method for internal performance evaluation in hospitals, but it also provided data support and decision-making references for hospital managers to comprehensively understand the medical service capabilities and efficiency of each department and improve the overall performance level.
