Analysis of clinical TNM staging assessment and management before tumor treatment in a cancer hospital in Shenzhen
Objectives To investigate and analyze the effect of the special improvement of pre-treatment clinical TNM staging assessment for five key cancer types in an oncology hospital,and provide feasible experiences to improve the pre-treatment clinical TNM staging assessment rate.Methods The discharged medical records of lung cancer,gastric cancer,colorectal cancer,liver cancer,and breast cancer patients who were first hospitalized in the hospital from January 1st to March 31st,2021(257),January 1st to March 31st,2022(193),and January 1st to March 31st,2023(263),were randomly selected according to the discharge time order,and the chi-square test was used to compare the effect of the improvement measures on the clinical TNM staging assessment before tumor treatment.Results Among the 713 medical records,the rate of completion of clinical stage assessment before treatment was 62.13%,among which the rate of non-surgical treatment(81.23%)was higher than that of surgical treatment(41.18%)(P<0.001).The evaluation rate after the implementation of the special action(2022:82.90%,2023:67.30%)was higher than that before the implementation of the special action(2021:41.25%)(P<0.001).Conclusions By carrying out special improvements,the assessment record rate was improved,and the non-surgical departments were higher than the surgical departments.It was necessary to further improve the reminder function of the electronic medical record system,implement quality control supervision feedback and performance assessment measures,and gradually improve the clinical TNM staging assessment rate before tumor treatment.
