Costs and Efficiency Analysis of Ischemic Cerebral Infarction Cases under DRGs Payment
Objectives Through investigating the changes in cost structure and medical efficiency of ischemic stroke cases before and after DRGs payment,analyze the impact of DRGs payment on medical costs and efficiency,and provide decision-making basis for hospital managers and policy makers.Method Taking ischemic stroke as an example,837 cases settled from March 15th,2021 to March 14th,2023 were extracted.Based on the actual time of DRGs in medical insurance,the cases were grouped and compared before and after the reform.The implementation effect of DRGs payment reform was analyzed using rank sum test.The disease costs and length of stay data of ischemic stroke before and after DRGs payment in a tertiary hospital in Beijing were statistically analyzed.Result It was found that the structure of medical costs was optimized,and the average cost of drugs per case decreased from 2861.85 yuan to 2437.09 yuan,and the median costs of drugs decreased from 2096.63 yuan to 1660.04 yuan,both of which showed significant statistical differences(P<0.05).After the reform,the proportion of patients with hospitalization days within 10 days increased from 69.49%to 73.91%.Conclusions After the implementation of DRGs payment,hospitals had increased their awareness of costs control and improved operational efficiency by optimizing the cost structure and shortening hospitalization time.
