摘要

目的 探讨中药红花黄色素A对重型颅脑损伤后炎性反应及预后的影响.方法 60例重型颅脑损伤病例,按照格拉斯哥昏迷评分(GCS)量表进行损伤程度分级,采取红花黄色素A干预治疗,分为干预组和对照组.酶联免疫吸附试验(ELISA)方法检测检测第0、3、6天炎性因子白细胞介素(IL)-6、IL-8和肿瘤坏死因子-α(TNF-α)的表达,格拉斯哥预后评分(GOS)评估治疗前后神经功能变化,分析红花黄色素A对重型颅脑损伤炎症和预后的关系.计量资料符合正态分布,以均数±标准差(x-±s)表示,采用t检验;计数资料采用频数表示,采用卡方检验.结果 与轻度颅脑损伤比较,中度和重度颅脑损伤的IL-6、IL-8、TNF-α表达水平高于轻度颅脑损伤,差异有统计学意义(x2=5.231,P<0.05).检测第0、3、6天3个时间点IL-6、IL-8、TNF-α表达水平,结果显示,随着损伤时间延长,两组的炎性因子表达水平逐步升高,差异有统计学意义(x2=7.421,P<0.05).与对照组比较,干预组第3天及第6天时间点的IL-6、IL-8、TNF-α表达水平低于对照组,差异有统计学意义(x2=4.370,P<0.05).在干预组中,与疗效差比较,疗效好的炎性因子IL-6、IL-8、TNF-α的表达水平低于疗效差组,差异有统计学意义(x2=8.140,P<0.05).结论 红花黄色素A能抑制促炎因子IL-6、IL-8、TNF-α表达水平,改善重度颅脑损伤预后.

Abstract

Objective To explore the effect of traditional Chinese medicine safflower yellow pigment A on inflammatory response and prognosis after severe traumatic brain injury.Methods A total of 60 cases of severe traumatic brain injury were classified according to the Glasgow Coma Scale(GCS)and treated with safflower yellow A intervention,divided into intervention group and control group.Enzyme linked immunosor-bent assay(ELISA)method for detecting inflammatory factors IL-6,IL-8,and TNF-αon days 0,3,and 6.The Glasgow Outcome Scale(GOS)was used to evaluate the changes in neurological function before and after treatment,and to analyze the relationship between safflower yellow pigment A and inflammation and prognosis in severe traumatic brain injury.The measurement data conforms to a normal distribution,represented by mean±standard deviation(x±s),and t-test is used;Count data is represented by frequency and chi square test is used.Results Compared with mild traumatic brain injury,the levels of IL-6,IL-8,and TNF-αin moderate and severe traumatic brain injury were higher.The expression level is higher than that of mild trau-matic brain injury,and the difference is statistically significant(x2=5.231,P<0.05).Detect IL-6,IL-8,and TNF-αat three time points d0,d3,and d6.The expression level showed that with the prolongation of in-jury time,the expression levels of inflammatory factors in the two groups gradually increased,and the differ-ence was statistically significant(x2=7.421,P<0.05).Compared with the control group,IL-6,IL-8,TNF-α at time points d3 and d6 in the intervention group.The expression level is lower than that of the con-trol group,and the difference is statistically significant(x2=4.370,P<0.05).In the intervention group,the inflammatory factors IL-6,IL-8,and TNF-α with better efficacy compared to those with poorer efficacy were found to be effective.The expression level of is lower than that of the treatment group with poor effica-cy,and the difference is statistically significant(x2=8.140,P<0.05).Conclusion Safflower yellow pig-ment A can inhibit pro-inflammatory factors IL-6,IL-8,and TNF-αof expression level improves the prognosis of severe traumatic brain injury.
