FK1706对神经源性膀胱大鼠膀胱形态恢复的作用

目的 探讨FK1706对神经吻合后神经源性膀胱(NB)大鼠膀胱形态恢复的影响.方法 40只健康成年雄性SD大鼠(河南省实验动物中心提供)按照随机数字表法分为FK1706+神经端侧吻合组(FK1706+ECG,15只)、神经端侧吻合组(ECG,15只)和手术对照组(SCG,10只).FK1706+ECG大鼠先离断左侧腰6(L6)和骶1(S1)脊神经前后根,然后将左侧L6前根端侧吻合到左侧L4前根上,术后连续8周给予皮下注射FK1706溶液(0.32 mg/kg);ECG大鼠手术方法同FK1706+ECG大鼠,术后连续8周注射相同剂量的30%的二甲基亚砜(DMSO);SCG大鼠离断左侧L6后根和S1前后根,作为手术对照组,术后连续8周注射相同剂量的30%的DMSO.术后4个月,麻醉3组大鼠,打开下腹部,暴露膀胱,取出膀胱并称重,剪开膀胱并测量膀胱壁厚度,比较3组大鼠膀胱湿重及膀胱壁厚度.之后将膀胱标本置于10%的福马林中,并放在4 ℃冰箱保存,将膀胱标本制作成石蜡切片,然后行苏木精-伊红(HE)和苦味酸-酸性品红(VG)染色,显微镜下观察并拍照,进行图像分析,计算并比较3组大鼠膀胱纤维化面积.两样本均数比较采用t检验.结果 FK1706+ECG组大鼠膀胱湿重[(0.291±0.023)g]和膀胱壁厚度[(1.613±0.051)mm]明显高于SCG组[(0.122±0.017)g 和(1.169±0.035)mm,t=19.463、23.537,P<0.01],但低于 ECG组[(0.415± 0.032)g 和(2.005±0.075)mm,t=11.345、15.583,P<0.01].HE 染色显示:SCG 组大鼠膀胱组织形态正常,上皮扁平,肌束排列整齐,肌束间结缔组织紧密;FK1706+ECG组和ECG组大鼠膀胱肌层排列不规则,并且有水肿,但FK1706+ECG组大鼠的肌层排列不规则和水肿程度较ECG组轻.VG染色显示:FK1706+ECG大鼠和ECG大鼠膀胱纤维化面积[(16.99±2.45)%比(19.99± 2.55)%]明显大于 SCG 大鼠膀胱纤维化面积[(11.10±1.38)%,t=6.796、9.928,P<0.01],但是FK1706+ECG大鼠膀胱纤维化程度较ECG大鼠轻(t=3.059,P<0.01).结论 FK1706能够促进神经吻合后NB大鼠膀胱形态的恢复.
Effect of FK1706 on bladder morphology recovery in rats with neurogenic bladder
Objective To investigate the effect of FK1706 on bladder morphology recovery after end-to-side neurorrhaphy in rats with neurogenic bladder(NB).Methods A total of 40 rats were random-ly divided into the FK1706 and end-to-sidc nerve coaptation group(FK1706+ECG,n=15),end-to-side nerve coaptation group(ECG,n=15),and surgical control group(SCG,n=10).In the FK1706+ECG,the left L6 and S1 ventral root(VR)and dorsal root(DR)were transected,and the distal stump of the L6 VR was sutured to the lateral face of the L4 VR.The ECG was subjected to the same procedure as the FK1706+ECG.In the SCG,the left L6 DR,S1 VR and S1 DR were transected.After the operation,the FK1706+ECG was treated with FK1706(0.32 mg/kg)dissolved in dimethyl sulfoxide(DMSO)for 8 weeks,and the ECG and SCG were treated with the same volume of DMSO.At 4 th month after operation,the bladder was removed,the wet weight of the bladder was measured and the thickness of the bladder wall was measured.Thereafter,the bladder specimens were placed in 10%formalin and stored in the refrigera-tor at 4 ℃.The bladder specimens were made into paraffin sections,and then stained with hematoxylin and eosin(HE)and VG,observed and photographed under the microscope.The area of bladder fibrosis in the three groups rats was calculated and compared.T-test was used for comparison between two groups.Results Bladder wet weight and bladder wall thickness were greater in the FK1706+ECG[(0.291± 0.023)g and(1.613±0.051)mm]than in the SCG[(0.122±0.017)g and(1.169±0.035)mm,t=19.463,23.537,P<0.01],but significantly lower than in the ECG[(0.415±0.032)g and(2.005± 0.075)mm,t=11.345,15.583,P<0.01].HE staining showed that the bladder morphology in the SCG was normal,the epithelium was flat,the muscle bundles were arranged neatly,and the connective tissue between the muscle bundles was tight.The detrusor in the FK1706+ECG and ECG was irregular and ede-ma,but the detrusor in the FK1706+ECG was less irregular and edema than that in the ECG.According to VG staining,the bladder fibrosis areas in FK1706+ECG and ECG[(16.99±2.45)%vs.(19.99± 2.55)%]were significantly larger than those in SCG[(11.10±1.38)%,t=6.796,9.928,P<0.01],but the degree of bladder fibrosis in FK1706+ECG was less than that in ECG(t=3.059,P<0.01).Conclusion FK1706 promotes bladder morphology recovery after end-to-side neurorrhaphy in rats with neurogenic bladder.
