Analysis of related factors of bladder neck contracture after transurethral resection of prostate
Objective To explore the related factors of bladder neck contracture(BNC)after tran-surethral resection of the prostate(TURP).Methods The clinical data of patients with benign prostatic hyperplasia(BPH)who were treated with TURP in the Department of Urology in our Hospital from January 2012 to December 2020 were collected.According to the occurrence of BNC after operation,the patients were divided into BNC group and non-BNC group.Then univariate and multivariate analysis was carried out by Logistic regression model to screen the independent risk factors of BNC after TURP.Results A total of 451 patients with BPH were enrolled in this study.The median follow-up time was 25.3(14.1-104.0)months,including 30 patients in the BNC group and 421 patients in the non-BNC group.Univariate and Multivariate Logistic regression analysis showed that prostate volume≤50 ml[odds ratio(OR)=2.890,95%confidence interval(CI):1.176-7.104,P<0.05],fibrinogen level ≤2.5 g/L(OR=2.246,95%CI:1.016-4.965,P<0.05)and postoperative indwelling catheter time ≥ 7 days(OR=2.850,95%CI:1.173-6.926,P<0.05)were independent risk factors for BNC after TURP.Conclusion In order to re-duce the occurrence of BNC after TURP,the level of fibrinogen should be determined before operation,the indication of operation should be strictly grasped for small prostate,and the urinary catheter should be re-moved as soon as possible when the urine color is clear after operation.The follow-up,early diagnosis and treatment of BNC should be strengthened within one year,so as to improve the surgical effect and quality of life of the patients.
