The difference study between calcaneus anatomy angle and heel pain diagnosis and treatment
Objective To investigate the relationship between calcaneal anatomical angle and cal-caneal pain and the relationship between calcaneal anatomical angle and bone cement injection in treating calcaneodynia.Methods A total of 284 adults with an mean age of 54 years who met the research condi-tions were selected from the patients who underwent calcaneal X-ray examination in the outpatient depart-ment from September 2019 to December 2022.The patients were divided into ordinary adult group(222 pa-tients)and heel pain disease group(62 patients).In order to investigate the relationship between calcaneal anatomical angle and calcaneal pain,the Bohler's angle,Gissane's angle and calcaneal inclination angle were measured according to the clinical manifestations,and the calcaneal anatomical angles were analyzed.Out of 50 of 62 patients in the heel pain group(12 patients were not given continuous treatment and fol-lowed up),18 patients were treated surgically with pain point injection of bone cement,and 32 patients with heel pain were treated conservatively.The Bohler's angle,Gissane's angle and calcaneal inclination angle of the 50 patients were measured according to the clinical manifestations,and the calcaneal anatomi-cal angles of the 50 patients were analyzed,to investigate the relationship between calcaneal anatomical an-gle and calcaneal pain in the patients with pain point injection of bone cement and conservation.Differ-ences between two groups were analyzed using the independent sample t-test.Results None of the com-parative differences were statistically significant in the male population,the Bohler angle[(31.23± 7.95)°],Gissane's angle[(117.03±5.86)°]and the control group[(32.27±6.96)°,(119.58± 8.25)°,(19.72±4.93)°,P>0.05].In the female population,there was no significant difference in the Bohler angle between the heel pain group[(33.52±6.04)°]and control group[(32.95±5.93)°,P>0.05];the Gissane's angle in the heel pain group[(114.37±6.52)°]was lower than the control group[(123.04±7.03)°,P<0.05],and the inclination angle in the heel pain group[(26.02±6.23)°]was higher than in the control group[(20.02±3.52)°,P<0.05].There were no significant differences in the calcaneal Bohler angle,Gissane's angle and inclination angle between the bone cement injection group[(31.89±5.93)°,(117.32±5.23)°,(21.93±5.23)°]and conservative group[(30.62±6.89)°,(115.83±4.96)°,(23.02±6.03)°,P>0.05].Conclusion In women,the smaller the Gissane's an-gle and the larger calcaneal inclination angle,the more likely they are to have heel pain.Clinical injection of bone cement pain points can not be focused on the anatomical angle of calcaneus.
