血清微小RNA-129-5p对骨关节炎的诊断价值及其与高迁移率族蛋白B1炎性信号通路的关系

目的 探讨血清微小RNA-129-5p(miR-129-5p)在骨关节炎(OA)中的表达水平及诊断价值.方法 选取2019年3月至2023年7月在山西省人民医院骨科就诊的126例OA患者为观察组,并纳入同期体检中心的105例健康体检者为对照组.采用实时定量反转录聚合酶链反应(RT-qPCR)检测血清miR-129-5p的表达水平;采用酶联免疫吸附试验(ELISA)检测血清高迁移率族蛋白B1(HMGB1)、肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)和C反应蛋白(CRP)的表达水平;利用Pearson相关系数分析miR-129-5p、HMGB1水平与TNF-α、IL-1β、CRP的相关性;分析miR-129-5p与HMGB1的相关性;利用多因素Logistic回归分析miR-129-5p对OA的风险性;并采用受试者工作特征(ROC)曲线判断miR-129-5p对OA的诊断价值.结果 观察组血清miR-129-5p水平显著低于对照组(0.49±0.28比0.99±0.34,t=12.390,P<0.01),观察组血清HMGB1水平显著高于对照组[(45.68±18.41)ng/ml 比(23.20±8.92)ng/ml,t=11.414,P<0.01].观察组血清TNF-α、1L-1β、CRP 水平均显著高于对照组(t=7.331、5.246、8.622,P<0.01).血清 miR-129-5p 水平与HMGB1呈明显负相关(r=-0.615,P<0.01),多因素Logistic回归分析结果显示,miR-129-5p的比值比(OR)值为0.019.ROC曲线分析显示,血清miR-129-5p截断值0.928对预测OA具有良好的灵敏度(95.24%)和特异性(59.05%),曲线下面积(AUC)为0.861[95%置信区间:0.815~0.907,P<0.01].结论 血清miR-129-5p水平在OA患者中显著降低,且与HMGB1及炎性标志物TNF-α、IL-1β、CRP呈负相关,对预测OA具有诊断价值.
The diagnostic value of serum microRNA-129-5p in osteoarthritis and its relationship with high mobility group protein B1 inflammatory signaling pathway
Objective To investigate the expression level and diagnostic value of serum microRNA-129-5p(miR-129-5p)in osteoarthritis(OA).Methods A total of 126 OA patients who were treated in the Department of Orthopedics of Shanxi Provincial People's Hospital from March 2019 to July 2023 were selected as the observation group,and 105 healthy people who underwent physical examination in the phys-ical examination center during the same period were included as the control group.Real-time fluorescent polymerase chain reaction(RT-qPCR)was used to detect the expression level of serum miR-129-5p.The expression levels of high mobility group protein B1(HMGB1),tumor necrosis factor-α(TNF-α),interleu-kin-1β(IL-1β)and CRP in serum were detected by enzyme-linked immunosorbent assay(ELISA).Pear-son correlation coefficient was used to analyze the correlation between miR-129-5 p,HMGB1 levels and TNF-α,IL-1β,CRP.The correlation between miR-129-5p and HMGB1 was analyzed.Multivariate Logis-tic regression was used to analyze the risk of miR-129-5p on OA.The receiver operating characteristic(ROC)curve was used to determine the diagnostic value of miR-129-5p in OA.Results The serum level of miR-129-5p in the observation group was significantly lower than that in the control group(0.49±0.28 vs.0.99±0.34,t=12.390,P<0.01).The serum level of HMGB1 in the observation group was significantly higher than that in the control group[(45.68±18.41)vs.(23.20±8.92)ng/ml,t=11.414,P<0.01].The serum levels of TNF-α,IL-1 β and CRP in the observation group were significantly higher than those in the control group(t=7.331,P<0.01;t=5.246,P<0.01;t=8.622,P<0.01).Serum miR-129-5p level was negatively correlated with HMGB1(r=-0.615,P<0.01).Multivariate Logistic regression a-nalysis showed that the odds ratio(OR)value of miR-129-5p was 0.019.The ROC curve analysis showed that the cut-off value of serum miR-129-5p was 0.928,which had good sensitivity(95.24%)and specific-ity(59.05%)in predicting OA,and the area under the curve(AUC)was 0.861[95%confidence inter-val:0.815-0.907,P<0.01].Conclusion The serum level of miR-129-5p is significantly decreased in OA patients,and it is negatively correlated with HMGB1 and inflammatory markers TNF-α,IL-1β and CRP,and has a good diagnostic value for predicting OA.
