Changes and clinical significance of bone metabolism indexes in patients with osteoporotic fracture
Objective To investigate the changes and clinical significance of bone metabolism in patients with osteoporotic fracture.Methods A total of 65 patients with postmenopausal osteoporotic frac-ture admitted to Zhumadian Hospital and the First Affiliated Hospital of Zhengzhou University from Decem-ber 2022 to May 2023 were selected as the study objects,and 65 patients with postmenopausal osteoporosis without fracture were selected as the controls.Venous blood was drawn in the morning,and the serum lev-els of 25-hydroxyvitamin D(25-OH-VitD),bone alkaline phosphatase(BLAP),osteocalcin(OC),typeⅠ collagen carboxyl terminal peptide(CTx),type Ⅰ procollagen amino terminal propeptide(PINP),par-athyroid hormone(PTH),insulin-like growth factor 1(IGF-1)and calcium and phosphorus values were determined by chemiluminescence method and enzyme link immunosorbent assay.The correlation between fracture and serum metabolic indexes of patients was analyzed.The t test was used to compare measurement data between groups.Results Bone mineral density in neck of femur,greater trochanter of femur,lumbar spine and Ward triangle of osteoporosis patients[(0.53±0.03),(0.51±0.04),(0.59±0.04),(0.60±0.11)g/cm2]were significantly higher than patients with osteoporotic fractures[(0.48±0.04),(0.47±0.05),(0.52±0.04),(0.50±0.05)g/cm2,t=6.628,5.670,10.600,6.479,P<0.05].The levels of25-OH-VitD and IGF-1 in osteoporosis patients[(25.91±1.85),(164.95±14.16)ng/ml]were significantly higher than those in osteoporotic fracture patients[(19.05±2.06),(118.02±14.13)ng/ml,t=19.950,18.770,P<0.05].The levels of BLAP,CTx,PINP,PTH,OC and blood phosphorus in patients with osteoporosis[(3.09±0.99),(0.29±0.04),(35.38±5.17),(48.97±8.81),(14.54±1.47),(1.11±0.08)ng/ml]were significantly lower than those with osteoporotic frac-tures[(4.84±1.44),(0.55±0.10),(51.71±8.80),(57.49±7.18),(21.54±4.05),(0.97±0.04)ng/ml,t=8.082,19.040,12.900,6.046,13.090,12.950,P<0.05].The results showed that BLAP,CTx,PINP,PTH,OC and serum phosphorus levels were positively correlated with osteoporotic fractures(B=0.054,0.068,0.121,0.102,0.102,0.204,P<0.05).Levels of 25-OH-VitD and IGF-1 were negatively correlated with osteoporotic fractures(B=-0.241,-0.312,P<0.05).Conclusion Biochemical markers of bone metabolism 25-OH-VitD,BALP,BGP,CTx,PINP,PTH,OC,IGF-1 and serum phosphorus are closely related to osteoporotic fractures,and are risk factors for osteoporotic frac-tures.
