用于治疗椎间盘退变的小分子和生物材料研究进展

椎间盘退变(IDD)是引起慢性腰腿痛的主要原因,如何延缓和治疗IDD 一直是临床的难题。近年来,许多小分子在基础研究中展现出良好的IDD治疗效果,但是由于IDD涉及髓核细胞(NPCs)、干细胞、成纤维细胞等多种细胞,并且包含炎症、凋亡等多种因素,因此其机制非常复杂,也使得这些小分子尚未能成功应用于临床。此外,椎间盘的血供较差,降低了药物维持有效浓度的持续时间,因此对药物载体的改良也是治疗IDD的重要一环。本文总结了近年来诸多学者发现的可能用于治疗IDD的小分子、生物材料及研究结果显示的机制,希望为后续的研究提供参考。
Research progress in small molecules and biomateials for treating intervertebral disc degeneration
Intervertebral disc degeneration(IDD)is the main cause of chronic low back pain,and how to de-lay and treat IDD has been a clinical challenge.In recent years,many drugs have shown good therapeutic effects on IDD in basic research.However,due to the involvement of various cells such as nucleus pulposus cells(NPCs),stem cells,fibroblasts,and other factors such as inflammation and apoptosis,the mechanism of IDD is very complex,which also makes these drugs have not been successfully applied in the clinic.In addition,the poor blood supply to interver-tebral discs reduces the duration of maintaining effective drug concentrations,so improving drug carriers is also an im-portant part of treating IDD.This article summarizes the potential drugs,biomaterials,and mechanisms discovered by many scholars in recent years for the treatment of IDD,hoping to provide reference for subsequent research.
