废用性骨质疏松的临床与发生机制研究进展

废用性骨质疏松症属于继发性骨质疏松症的一种,临床上各种疾病需要制动或者失重状态(航空、航天等),抑或个别肢体活动量不足甚至完全失用是该疾病发生、发展的危险因素,继而可能导致废用性骨质疏松相关的一系列并发症,如快速骨丢失、复杂局部疼痛综合征、关节周围软组织骨化(异位骨化)、肢体畸形以及病理性骨折等,严重影响患者原发疾病的康复和日常生活.目前,随着骨质疏松发病机制和临床治疗药物研发的不断深入,废用性骨质疏松症日益引起医学界越来越多的关注.本文就近年来在该领域临床流行病学的研究进展,不同类型废用性骨质疏松的临床表现、骨代谢改变及其评价方法,以及失用导致骨丢失的发病机制作一概述,以期探讨废用性骨质疏松症的致病原因、发病机理和寻找有效的防治措施.
Advances in clinical and mechanism of disuse osteoporosis
Disuse osteoporosis is a type of secondary osteoporosis.Clinically,various diseases re-quire immobilization or weightlessness(aviation,aerospace,etc.),or insufficient physical activity of indi-vidual limbs or even complete disuse are risk factors for the occurrence and development of this disease.It may lead to a series of complications related to disuse osteoporosis,such as rapid bone loss,complex re-gional pain syndrome,soft tissue ossification around the joint(ectopic ossification),limb deformities and pathological fractures,which seriously affect the recovery and daily life of patients.At present,with the development of the pathogenesis of osteoporosis and clinical treatment drugs,disuse osteoporosis has attrac-ted more and more attention.This paper reviews the recent progress of clinical epidemiology,the clinical manifestations of different types of disuse osteoporosis,bone metabolism and the evaluation methods,as well as the pathogenesis.In order to explore the causes of disuse osteoporosis,pathogenesis and effective prevention and treatment method.
