摘要

目的 探讨超重及肥胖2型糖尿病(T2DM)患者认知功能与脑结构影像的相关性.方法 为横断面研究.纳入2016年2月至2023年7月就诊于南京大学医学院附属鼓楼医院内分泌科的T2DM患者,并根据体重指数(BMI)将其分为正常体重T2DM组和超重及肥胖T2DM组,招募同期就诊于南京大学医学院附属鼓楼医院健康管理中心年龄和受教育时间与其相匹配且18.5 kg/m2≤BMI<24.0 kg/m2的健康者作为健康对照组.收集3组研究对象的年龄、受教育时间、身高和体重并计算BMI,测量其Android区域脂肪含量,并计算脂肪质量指数(FMI),采用可重复的成套神经心理状态测量量表评估即刻记忆,测量其左右侧海马体积.采用独立样本t检验或单因素方差分析进行组间比较,采用协方差分析比较3组间认知功能、总脑及皮层下核团体积的差异,采用中介效应模型分析人体脂肪分布、脑结构、认知功能之间的关系.结果 共纳入研究对象350例.其中,健康对照组50例,正常体重T2DM组100例,超重及肥胖T2DM组200例.与正常体重T2DM组相比,超重及肥胖T2DM组患者Android区域FMI更高(t=11.90,P<0.001).协方差分析结果显示,与健康对照组和正常体重T2DM组相比,超重及肥胖T2DM组患者即刻记忆得分均更低(P值分别为0.048和0.041)、左右侧海马体积均更小(健康对照组和超重及肥胖T2DM组,正常体重T2DM组和超重及肥胖T2DM组左右侧海马体积比较,左侧P值分别为0.002和0.036,右侧P值分别为0.004和0.022).中介效应模型分析结果显示,在T2DM患者中,左侧海马体积部分介导了 Android区域FMI与即刻记忆得分之间的相关性(β=-0.313,95%CI-0.695~-0.076).结论 超重及肥胖T2DM患者相较正常体重T2DM患者认知功能损害及海马体积萎缩更为显著.T2DM患者腹部脂肪增加与认知功能损害和海马体积萎缩有关.

Abstract

Objective To investigate the association between cognitive function and brain structure changes in overweight and obese patients with type 2 diabetes mellitus(T2DM).Methods This was a cross-sectional study.T2DM patients who visited the Endocrinology Department of Nanjing Drum Tower Hospital,Affiliated Hospital of Medical School,Nanjing University,from February 2016 to July 2023 were collected.T2DM Patients were divided into normal weight T2DM group,overweight and obese T2DM group according to body mass index(BMI).At the same time,we recruited healthy people who visited the Health Management Center of Nanjing Drum Tower Hospital,Affiliated Hospital of Medical School,Nanjing University,matched for age and education level,and whose 18.5 kg/m2 ≤BMI<24.0 kg/m2 served as the healthy control group.The age,education level,height and weight of the three groups were collected,and BMI was calculated.The fat mass in android area was measured to calculate the fat mass index(FMI).Immediate memory was assessed using Repeatable Battery for the Assessment of Neuropsychological Status.Left and right hippocampal volumes were measured.Independent-samples t test and one-way analysis of variance were used to compare data between the groups.Covariance one-way analysis of variance was used to compare cognitive function,the volume of total brain and subcortical nuclei.The mediation effect model was used to analyze the mediating effect of brain structure on the relationship between body fat distribution,brain structure and cognitive function.Results A total of 350 people were enrolled,including 50 healthy controls,100 normal weight T2DM patients,200 overweight and obese T2DM patients.Compared with normal weight T2DM group,overweight and obese T2DM patients have higher FMI in android area(t=11.90,P<0.001).The results of covariance one-way analysis of variance showed that compared with healthy controls and normal weight T2DM patients,overweight and obese T2DM patients have lower immediate memory score(P values were 0.048 and 0.041,respectively)and significantly reduced left and right hippocampal volume(P values were 0.002 and 0.036 on the left side and P values were 0.004 and 0.022 on the right side,respectively).Mediation effect model analysis showed that in patients with T2DM,the volume of left hippocampus partially mediated the association between FMI of android area and immediate memory score(β=-0.313,95%CI-0.695--0.076).Conclusions Cognitive impairment and hippocampal volume atrophy were more significant in overweight and obese T2DM patients than in normal weight T2DM patients.Increased abdominal fat in T2DM patients is associated with cognitive impairment and hippocampal volume atrophy.
