摘要
大椎穴是临床上疾病诊治最常用腧穴之一，历代医家在文献著作中均有记载和论述，但其中对于大椎定位及取穴的描述却有多种不同解读，形成了不同的临床取穴方法，直接影响疗效。通过分析古代文献、现代高等中医药院校教材、国家标准中关于大椎穴定位及取穴的描述，探析大椎穴定位与取穴的认知演变过程。结合现代解剖学确定其标准定位及取穴，大椎穴的定位应为第7颈椎棘突与第1胸椎棘突之间的凹陷中，俯头取之，且“俯头取之”为大椎穴定位取穴之关键。 Dazhui(GV14) is one of the most commonly used acupoints in clinical diagnosis and treatment of diseases, which has been recorded and discussed in the literature by generations.However, there are many different interpretations of the description of the location of Dazhui(GV14), forming different acupoint selection methods, which directly affect the efficacy.Taking this as a problem-oriented approach,we analyzed the cognitive changes of the location of Dazhui(GV14) and acupoint selection by analyzing the descriptions of Dazhui(GV14) and acupoint selection in ancient literature, textbooks of modern Chinese medicine colleges and national standards.Combining modern anatomy and a variety of different perspectives,we believe that Dazhui(GV14) should be in the depression between the spinous process of the 7th cervical vertebra and the spinous process of the 1st thoracic vertebra, and looking down to take the acupoint is the key of its needling techniques.
