骨科住院患者围手术期抗菌药物使用评价

目的 对骨科住院患者围手术期抗菌药物的使用情况进行分析评价,评估其用药合理性.方法 提取医院骨科2008年9～11月手术患者病历267份,运用药物利用评价及药物频度分析法,对抗菌药物应用情况进行回顾性统计分析与评价.结果 267例手术患者全部使用了抗菌药物,使用居前的品种几乎全是头孢菌素类抗菌药物,用药频度较高的药品是头孢米诺、头孢美唑、头孢呋辛,日限定剂量(DDD)费用和药品总费用最高的品种为头孢孟多、氟氧头孢等;清洁手术、清洁-污染手术、污染手术平均使用抗菌药物天数分别为7、8、9 d;联合用药86%.结论 骨科围手术期抗菌药物的使用存在不合理之处,应遵守用药指南,提高整体用药水平.
Abstract
OBJECTIVE To analyze and review the antibiotics utilization for the perioperative inpatients in our orthopedics department, and to evaluate the rationality of the utilization. METIIODS Totally 267 cases of inpatients receiving operations from Sep to Nov, 2008 were selected in our orthopedics department, then the methods of drug use review and drug frequency analysis were applied to analyze and review the effects of antibiotics utilization with retrospective statistics. RESULTS All of the 267 inpatients have taken antibiotics, of which the cephalosporins were the most frequently used. The antibiotics commonly used were cefminox,cefmetazole, and cefuroxime. The antibiotics of the highest daily defined dose (DDD) costs and total costs were cefamandole, flomoxef, etc. The average days of antibiotics utilization for clean operations, clean-contaminated operations, and contaminated operations were 7 days, 8 days, and 9 days, respectively and 86% of the patients were antibiotic combination usage. CONCLUSIONS There is inappropriateness in the antibiotics utilization during perioperations of orthopedics. Guidelines of drug use must be followed to improve the drug usage.
