医护人员洗手依从性与感染相关性调查

摘要
目的：研究医护人员洗手依从性和医院感染的相关性，为临床防治感染提供参考。方法调查2014年1月－2015年1月在岗医护人员的洗手依从性，每日由经过专业培训的两名研究人员对医师、护士进行有目的连续观察3 h ，记录医护人员的洗手次数，计算洗手率；同时选取同期住院治疗420例患者作为医院感染观察对象，根据感染情况分为感染组与未感染组，观察两组患者发生医院感染情况；对医护人员洗手依从性与患者医院感染发生率进行相关分析。结果医护人员在院内各项活动洗手依从性均＜50．00％，护理人员的洗手依从性显著好于医师；不同职称医务人员洗手合格率以主治医师、主管护师最高，实习护士最低；420例患者中发生感染22例，感染率为5．24％；其中感染组对应的医护人员洗手率为31．97％、洗手达标率为52．46％，未感染组则分别为57．32％、89．96％，两组差异有统计学意义（ P＜0．05）；医护人员洗手依从性与医院感染率呈显著线性负相关。结论医护人员洗手依从性与医院感染率呈显著线性负相关，应采取多项积极措施提高医护人员洗手依从性，从而降低患者医院感染发生率。
Abstract
OBJECTIVE To investigate the relevance of medical staff hand hygiene compliance and hospital infection so as to provide reference for clinic prevention of infections .METHODS From Jan .2014 to Jan .2015 ,the hand hygiene compliance of on duty physicians and nurses were observed for 3 hours intentionally by two well trained researchers daily .The times of hand hygiene were recorded to figure out hand hygiene rate .In the same period , 420 cases of hospitalized patients were chosen as infection objects .They were divided into infection group and non‐infected group according to their infection condition .The relevance of medical staff hand hygiene compliance and hospital infection rates were analyzed .RESULTS The hand hygiene compliance rates of medical staff at hospital were all less than 50% .Nurses had higher rates than the physicians .Attending physicians and supervisor nurses got the highest compliance rates and practicing nurses got the lowest .Among 420 patients ,22 were infected and the infection rate was 5 .24% .Medical staff in charge in the infection group had the hand hygiene compliance rate of 31 .97% and hand hygiene qualified rate of 52 .46% ,while the non‐infected group got the rates of 57 .32% and 89 .96% .The differences were significant (P<0 .05) .The medical staff hand hygiene compliance was negatively related to the hospital infection rate .CONCLUSION The medical staff hand hygiene compliance was negatively re‐lated to the hospital infection rate .More measures should be taken to improve the hygiene compliance of medical staff to decrease hospital infection rate .
