目的 探讨慢性心力衰竭合并肺部感染病原菌分布及血清单核细胞趋化蛋白-1(MCP-1)、微小核糖核酸-21(miR-21)及长链非编码核糖核酸(lncRNA)核富含丰富的转录本1(NEAT1)的预测价值。方法 选取2021年4月-2023年1月河南大学淮河医院心血管内科收治的慢性心力衰竭患者403例,根据是否并发肺部感染分为感染组104例和未感染组299例,分析慢性心力衰竭合并肺部感染病原菌分布特点,受试者工作特征(ROC)曲线分析MCP-1、miR-21、lncRNA NEAT1对慢性心力衰竭合并肺部感染的预测价值。结果 104例慢性心力衰竭合并肺部感染患者检出菌株125株,其中以革兰阴性菌为主,占比为76。00%;感染组患者血清MCP-1、miR-21、lncRNA NEAT1水平高于未感染组(P<0。05);绘制ROC曲线显示,血清MCP-1、miR-21、lncRNA NEAT1联合预测慢性心力衰竭合并肺部感染的曲线下面积(AUC)值为0。916,敏感度和特异度均为84。62%。结论 慢性心力衰竭合并肺部感染病原菌以革兰阴性菌为主,血清MCP-1、miR-21、lncRNA NEAT1呈高表达,且三者联合对慢性心力衰竭合并肺部感染的预测价值更高。
Etiological characteristics of chronic heart failure patients complicated with pulmonary infection and predictive values of MCP-1,miR-21,lncRNA NEAT1
OBJECTIVE To explore the distribution of pathogens isolated from the heart failure patients complicated with pulmonary infection and analyze the predictive values of serum monocyte chemoattractant protein-1(MCP-1),microribonucleic acid-21(miR-21)and long non-coding ribonucleic acid(lncRNA)nuclear rich tran-script 1(NEAT1).METHODS A total of 403 patients with chronic heart failure who were treated in cardiovascu-lar medicine department of Huaihe Hospital,Henan University from Apr 2021 to Jan 2023 were enrolled in the study and were divided into the infection group with 104 cases and the no infection group with 299 cases according to the status of pulmonary infection.The distribution of the pathogens isolated from the chronic heart failure pa-tients complicated with pulmonary infection was observed,and the values of MCP-1,miR-21 and lncRNA NEAT1 in prediction of the pulmonary infection were analyzed by means of receiver operating characteristic(ROC)curves.RESULTS Totally 125 strains of pathogens were isolated from the the 104 chronic heart failure patients complicat-ed with pulmonary infection,76.00%of which were gram-negative bacteria.The levels of serum MCP-1,miR-21 and lncRNA NEAT1 of the infection group were higher than those of the no infection group(P<0.05).ROC curve analysis showed that the area under curve(AUC)of the joint detection of serum MCP-1,miR-21 and ln-cRNA NEAT1 was 0.916 in prediction of the pulmonary infection in the patients with chronic heart failure,with the sensitivity 84.62%,the specificity 84.62%.CONCLUSION The gram-negative bacteria are dominant among the pathogens isolated from the chronic heart failure patients complicated with pulmonary infection.The patients show high expression levels of serum MCP-1,miR-21 and lncRNA NEAT1,and the joint detection of the three indexes has higher value in prediction of the pulmonary infection in the chronic heart failure patients.
