miR-146a和miR-181a及MIF诊断幽门螺杆菌相关胃黏膜炎症的临床价值

目的 探究微小RNA(miR)-146a、miR-181a和巨噬细胞移动抑制因子(MIF)在幽门螺杆菌(Hp)相关胃黏膜炎症中的表达及临床意义。方法 选取2020年5月-2023年1月江南大学附属医院收治的208例慢性胃炎患者为研究组,根据是否发生Hp感染分为Hp阳性组142例和Hp阴性组66例,并选取同期210名健康体检者为对照组,采集外周血检测miR-146a、miR-181a和MIF mRNA相对表达量,采用受试者工作特征(ROC)曲线分析miR-146a、miR-181a和MIF对Hp相关胃黏膜炎症的诊断价值。结果 研究组miR-146a、miR-181a和MIF mRNA相对表达量均高于对照组(P<0。05),Hp阳性组高于Hp阴性组(P<0。05);Hp阳性组上述指标mRNA相对表达量与胃痛、反酸、烧心、Hp感染史、饮食习惯、发病部位有关(P<0。05);ROC曲线结果显示,miR-146a、miR-181a和MIF联合检测诊断Hp相关胃黏膜炎症的临床价值较高,曲线下面积(AUC)、敏感度和特异度分别为0。908、88。70%和75。80%。结论 miR-146a、miR-181a和MIF在慢性胃炎及Hp相关胃黏膜炎症中呈现高表达,其在Hp相关胃黏膜炎症中高表达与多种因素有关,三者联合检测对Hp相关胃黏膜炎症的诊断价值较高。
Clinical values of miR-146a,miR-181a and MIF in diagnosis of Helicobacter pylori-associated gastric mucosal inflammation
OBJECTIVE To explore the expressions of microRNA(miR)-146a,miR-181a and macrophage migration inhibitory factor(MIF)in the patients with Helicobacter pylori-associated gastric mucosal inflammation and ana-lyze the clinical significance.METHODS Totally 208 patients with chronic gastritis who were treated in the Affilia-ted Hospital of Jiangnan University from May 2020 to Jan 2023 were assigned as the study group and were divided into the Hp-positive group with 142 cases and the Hp-negative group with 66 cases according to the status of H.pylori infection.Meanwhile,210 healthy people who received physical examination were chosen as the control group.Peripheral blood specimens were collected to detect the relative expression levels of miR-146a,miR-181a and MIF mRNA.The values of miR-146a,miR-181a and MIF in diagnosis of H.pylori-associated inflammation were analyzed by means of receiver operating characteristic(ROC)curves.RESULTS The relative expression lev-els of miR-146a,miR-181a and MIF mRNA of the study group were higher than those of the control group(P<0.05);the expression levels of the above indexes of the Hp-positive group were higher than those of the Hp-nega-tive group(P<0.05);the relative expression levels of mRNA of the above indexes of the Hp-positive group were associated with stomach pain,acid reflux,heartburn,history of H.pylori,dietary habit and pathogenesis site(P<0.05).The ROC curve analysis showed that the clinical value of the joint detection of miR-146a,miR-181a and MIF was high in diagnosis of H.pylori-associated gastric mucosal inflammation,with the area under curve(AUC)0.908,the sensitivity 88.70%,the specificity 75.80%.CONCLUSION The patients with chronic gastritis and H.pylori-associated gastric mucosal inflammation show high expression levels of miR-146a,miR-181a and MIF,and the high expressions of the above indexes in the patients with H.pylori-associated gastric mucosal in-flammation are associated with multiple factors,and the joint detection of the three indexes has high value in diag-nosis of the H.pylori-associated gastric mucosal inflammation.
