新生儿感染性肺炎血清sTREM-1、MMP-9、α1-AT、25(OH)D3水平与感染类型的关系

目的 分析新生儿感染性肺炎血清可溶性髓样细胞触发受体-1(sTREM-1)、基质金属蛋白酶-9(MMP-9)、α1-抗胰蛋白酶(α1-AT)、25羟维生素D3[25(OH)D3]水平与感染性肺炎新生儿感染类型的关系。方法 选取2021年1月-2022年12月简阳市人民医院收治的109例感染性肺炎新生儿作为感染组,选取同期在医院分娩的115名健康新生儿作为非感染组,比较两组血清sTREM-1、MMP-9、α1-AT、25(OH)D3水平,根据感染性肺炎新生儿感染类型分别将其分为细菌感染组(65例)、非细菌感染组(44例),采用受试者工作特征(ROC)曲线分析血清sTREM-1、MMP-9、α1-AT、25(OH)D3对感染性肺炎新生儿感染类型的鉴别价值。结果 感染组血清sTREM-1、MMP-9、α1-AT水平高于非感染组(P<0。05),血清25(OH)D3水平低于非感染组(P<0。05);细菌感染组血清sTREM-1、MMP-9、α1-AT水平高于非细菌感染组(P<0。05),血清25(OH)D3水平低于非细菌感染组(P<0。05);四项指标联合鉴别感染性肺炎新生儿感染类型的曲线下面积(AUC)值为0。866,检测的敏感度和特异度分别为81。54%和77。27%。结论 新生儿感染性肺炎的发生可引起血清sTREM-1、MMP-9、α1-AT、25(OH)D3水平变化,同时该四项指标对感染性肺炎新生儿感染类型具有较好的鉴别价值。
Serum sTREM-1,MMP-9,α1-AT and 25(OH)D3 of neonates with infectious pneumonia and association with types of pneumonia
OBJECTIVE To observe the levels of serum soluble triggering receptor expressed on myeloid cells(sTREM-1),matrix metalloproteinase-9(MMP-9),α1-antitrypsin(α1-AT)and 25 hydroxyvitamin D3[25(OH)D3]in the neonates with infectious pneumonia and analyze the association with the types of infectious pneumonia.METHODS A total of 109 neonates with infectious pneumonia who were treated in Jianyang People's Hospital from Jan 2021 to Dec 2022 were assigned as the infection group,meanwhile 115 healthy neonates who were born in the hospital were chosen as the non-infection group.The levels of serum sTREM-1,MMP-9,α1-AT and 25(OH)D3 were compared between the two groups.The neonates of the infection group were divided into the bacte-rial infection group with 65 cases and the non-bacterial infection group with 44 cases according to the types of in-fectious pneumonia.The values of serum sTREM-1,MMP-9,α1-AT and 25(()H)D3 in differential diagnosis of types of infectious pneumonia were analyzed by means of receiver operating characteristic(ROC)curves.RESULTS The levels of serum sTREM-1,MMP-9 and α1-AT of the infection group were higher than those of the non-infection group(P<0.05),while the serum 25(OH)D3 level of the infection group was lower than that of the non-infection group(P<0.05).The levels of serum sTREM-1,MMP-9 and α1-AT of the bacterial infection group were higher than those of the non-bacterial infection group(P<0.05),while the serum 25(OH)D3 level of the in-fection group was lower than that of the non-bacterial infection group(P<0.05).The area under curve(AUC)of the joint detection of the four indexes was 0.866 in differential diagnosis of the types of infectious pneumonia,with the sensitivity 81.54%,the specificity 77.27%.CONCLUSION The infectious pneumonia may lead to the changes of serum sTREM-1,MMP-9,α1-AT and 25(OH)D3 level of the neonates.The four indexes have significant value in differential diagnosis of the types of infectious pneumonia.
