目的 分析摩氏摩根菌临床分离株分布特点及其耐药性。方法 选取2013年1月-2020年12月南京医科大学第一附属医院临床分离出的276株摩氏摩根菌作为研究对象;剔除同一患者相同部位重复菌株;分析其基本信息、万人检出率变化趋势、标本来源、耐药性变化趋势。结果 276例摩氏摩根菌检出患者中,>60岁占63。41%;感染类型中由高到底分别为社区获得性感染(CAI,39。49%)、定植(37。68%)和医院获得性感染(HAI,22。83%);科室分布中,以外科(63。77%)为主,外科中泌尿外科(15。94%)和心脏大血管外科(12。32%)最多;2013-2020年摩氏摩根菌每万人检出率呈上升趋势(P<0。001);标本来源中以痰标本(39。49%)、中段尿(27。54%)和分泌物(10。51%)为主;以2年为单位,2013-2020年摩氏摩根菌的耐药趋势分析显示,其对哌拉西林/他唑巴坦、氨苄西林/舒巴坦及头孢吡肟呈下降趋势(P<0。05)。结论 摩氏摩根菌主要分布于外科科室,来源于痰标本和中段尿,检出率呈逐年上升趋势,对抗菌药物耐药率趋势整体较稳定,需合理选用抗菌药物,密切关注其耐药变化。
Distribution and drug resistance of Morganella morgeni
OBJECTIVE To analyze the distribution and drug resistance of Morganella morgeni isolates.METHODS Totally 276 strains of Morganella morgeni clinically isolated from the First Affiliated Hospital of Nanjing Medical University from Jan.2013 to Dec.2020 were selected as the study subject.Duplicate strains from the same site of the same patient were excluded.The basic information,the trend of 10 000-person detection rate,the source of specimens and the trend of drug resistance were analyzed.RESULTS Among the 276 patients with Morganella morgeni,63.41%were>60 years old;the types of infection were,from the highest to the lowest,community acquired infection(CAI,39.49%),colonization(37.68%)and hospital acquired infection(HAI,22.83%);the distribution of departments was dominated by surgery(63.77%),with urology(15.94%)and cardiac macrovas-cular surgery(12.32%)being the most prevalent among surgical departments;the detection rate of Morganella morgeni per 10 000 population showed an increasing trend from 2013 to 2020(P<0.001);among the sources of specimens,sputum specimens(39.49%),mid-stream urine(27.54%),and secretions(10.51%)were predomi-nant;analysis of the resistance trend of Morganella morgeni from 2013 to 2020 on a 2-year basis showed a de-creasing trend to piperacillin/tazobactam,ampicillin/sulbactam,and cefepime(P<0.05).CONCLUSION Mor-ganella morgeni was mainly distributed in surgical departments,originated from sputum specimens and mid-stream urine,the detection rate showed an increasing trend year by year,and the trend of antimicrobial resistance rate was relatively stable on the whole.Antibiotics should be selected reasonably and the change of drug resistance should be closely monitored.
