摘要
目的 探讨血清高尔基体糖蛋白73(GP73)和微小核糖核酸-122a(miR-122a)与miR-125b对脓毒症患者并发肝损伤的预测价值.方法 选取2019年1月-2023年3月镇江市第一人民医院收治的脓毒症并发肝损伤患者120例作为病例组,并根据肝损伤程度分为轻度组(42例)、中度组(44例)和重度组(34例),另选132例同期收治的单纯脓毒症患者作为对照组;比较病例组、对照组和不同肝损伤程度脓毒症并发肝损伤患者血清GP73、miR-122a、miR-125b水平,绘制受试者工作特征(ROC)曲线,分析血清GP73、miR-122a、miR-125b对脓毒症并发肝损伤的预测价值.结果 病例组血清GP73、miR-122a、miR-125b均高于对照组(P＜0.05);重度组血清GP73、miR-122a、miR-125b均高于中度组和轻度组(P＜0.05),中度组均高于轻度组(P＜0.05);血清GP73、miR-122a、miR-125b联合诊断脓毒症并发肝损伤的曲线下面积(AUC)为0.937,高于血清GP73、miR-122a、miR-125b单独检测(0.872、0.847、0.806,P＜0.05).结论 脓毒症并发肝损伤患者血清GP73、miR-122a、miR-125b水平呈高表达,三者水平变化参与患者的病情进展过程,且三者联合检测对脓毒症并发肝损伤的诊断价值更高.
Abstract
OBJECTIVE To investigate the predictive value of serum Golgi glycoprotein 73(GP73),microribonucle-ic acid 122a(miR-122a),and microribonucleic acid 125b(miR-125b)in sepsis patients with liver injury.METHODS Totally 120 patients with sepsis complicated by liver injury admitted to the First People's Hospital of Zhenjiang City from Jan.2019 to Mar.2023 were selected as the case group,and were divided into the mild group(42 cases),the moderate group(44 cases),and the severe group(34 cases)according to the degree of liver inju-ry,and 132 patients with simple sepsis admitted during the same period were selected as the control group;the serum levels of GP73,miR-122a,and miR-125b were compared among the case group,the control group and pa-tients with sepsis complicated with liver injury of different degrees of liver injury,and plotted the receiver operat-ing characteristic(ROC)curve,the predictive value of serum GP73,miR-122a,and miR-125b for sepsis compli-cated with liver injury were analyzed.RESULTS The serum GP73,miR-122a,and miR-125b in the case group were higher than the control(P＜0.05).The serum GP73,miR-122a,and miR-125b in the severe group were higher than the moderate and the mild groups(P＜0.05),the moderate group were all higher than the mild group(P＜0.05).The area under the curve(AUC)value of serum GP73,miR-122a,and miR-125b combined for the diagnosis of sepsis complicated with liver injury was 0.937,which was higher than that of serum GP73,miR-122a,and miR-125b detected separately(0.872,0.847,0.806,P＜0.05).CONCLUSION The Serum GP73,miR-122a,and miR-125b levels were highly expressed in patients with sepsis complicated by liver injury.The changes in levels of three were involved in the progression of the patients condition,and diagnostic value of the combined detection of the three was higher in sepsis-complicated liver injury.
