Hp infection in children with idiopathic dwarfism with gastrointestinal symptoms and its relationship with blood circulating ghrelin,TRX-1 and leptin levels
OBJECTIVE To investigate the Helicobacter pylori(Hp)infection in children with idiopathic dwarfism with gastrointestinal symptoms and its relationship with levels of blood circulating growth hormone releasing pep-tide(ghrelin),thioredoxin 1(TRX-1)and leptin(LP).METHODS Totally 127 children with idiopathic dwarfism with gastrointestinal symptoms admitted to the Affiliated Qingdao Central Hospital of Qingdao University be-tween Jan.2022 and Feb.2023 were selected as the study subjects,the children's Hp infection and its relationship with gastric mucosal lesion degree and inflammatory activity were counted,and the effects of Hp infection on the growth and development of children were analyzed.The children were divided into 56 cases in the infected group and 71 cases in the non-infected group according to the status of Hp infection,and the diagnostic value of ghrelin,TRX-1 and LP levels for Hp infection and the correlation with the degree of Hp infection were analyzed.The infec-ted group was given eradication treatment for Hp infection,and the patients were divided into effective group and ineffective group based on the clinical efficacy after 1 course of treatment.The value of the combined detection of indicators in assessing clinical efficacy was analyzed by receiver operating characteristic(ROC)curve.RESULTS The infection rate of gastric mucosa with moderate or severe lesions was higher than that of no or mild lesions,and the infection rate of moderate or severe inflammatory activity was higher than that of no or mild inflammatory activity,and the infection rate of children with Hp infection in family member was higher than that of children without Hp infection(P<0.05).The height,growth rate and growth inhibitory hormone levels of the infected group were lower than those of the non-infected group,and the body mass was smaller than that of the non-infec-ted group(P<0.05).The level of blood circulating ghrelin in infected group was lower than that in non-infected group,while the levels of serum TRX-1 and LP were higher than those in non-infected group(P<0.05).The area under the curve(AUC)values of the indicators combined detection for diagnosis of Hp infection were higher than that of blood circulating ghrelin,TRX-1 or LP alone(P<0.05).Blood circulating ghrelin was negatively correla-ted with C14-BUT value of Hp infection,while TRX-1 and LP levels were positively correlated with C14-BUT value of Hp infection(r=-0.774,0.672,0.587,P<0.05).After treatment,the level of blood circulating ghre-lin in effective group was higher than that in the ineffective group,while the levels of serum TRX-1 and LP were lower than those in ineffective group(P<0.05).The AUC value of combined detection of each indicator in evalua-ting therapeutic efficacy of Hp treatment was higher than that of blood circulating ghrelin,TRX-1 or LP alone(P<0.05).CONCLUSION Hp infection in children with idiopathic dwarfism with gastrointestinal symptoms was related to the degree of gastric mucosal lesions and inflammatory activity,and the combined detection of serum ghrelin,TRX-1 and LP levels had a diagnostic value for the assessment of Hp infection and the effect of Hp eradi-cation,and the changes of serum indicators were related to the degree of Hp infection.
