目的 分析2020-2022年苏州市区域性医院感染监测平台中央血管导管相关血流感染(CLABSI)监测数据。方法 对2020年1月-2022年12月苏州市区域性医院感染监测平台51所医院的CLABSI发病率、病原菌分布及耐药特征进行统计分析。结果 2020-2022年共发生CLABSI 1 903例次,发病率为0。332‰,其中三级医院高于二级医院(P<0。05);三级综合医院发病率高于专科医院;共培养分离病原菌1 268株,以肺炎克雷伯菌、表皮葡萄球菌、金黄色葡萄球菌、人葡萄球菌、鲍氏不动杆菌和铜绿假单胞菌为主;二级医院和三级医院病原菌分布较为一致;肺炎克雷伯菌、耐碳青霉烯类肺炎克雷伯菌(CRKP)、鲍氏不动杆菌和耐碳青霉烯类鲍氏不动杆菌(CRAB)仅对替加环素较为敏感;大肠埃希菌对头孢唑林、头孢呋辛、头孢曲松、头孢噻肟、左氧氟沙星、诺氟沙星、莫西沙星、环丙沙星和磺胺甲噁唑/甲氧苄啶的耐药率较高;铜绿假单胞菌对多数抗菌药物的耐药率<30。00%;金黄色葡萄球菌和耐甲氧西林金黄色葡萄球菌(MRSA)对红霉素、克林霉素的耐药率>53。39%;表皮葡萄球菌和耐甲氧西林表皮葡萄球菌(MRSE)对红霉素、克林霉素、左氧氟沙星、环丙沙星和磺胺甲噁唑/甲氧苄啶的耐药率为47。33%~73。08%。结论 本市不同等级和类别医院的CLABSI流行特征存在一定差异,应结合当地监测资料及时初始经验用药有效控制感染和减缓其传播。
Surveillance report of central line-associated bloodstream infection from regional nosocomial infection surveillance platform in Suzhou between 2020 to 2022
OBJECTIVE To analyze the surveillance data of central line-associated bloodstream infection(CLABSI)from regional nosocomial infection surveillance platform in Suzhou between 2020 to 2022.METHODS The inci-dence rate,pathogenic bacteria distribution and drug resistance characteristics of CLABSI in 51 hospitals of the re-gional nosocomial infection surveillance platform in Suzhou from Jan.2020 to Dec.2022 were statistically analyzed.RESULTS A total of 1 903 cases of CLABSI occurred from 2020 to 2022,with an incidence rate of 0.332‰,of which tertiary hospitals was higher than secondary hospitals(P<0.05).The incidence of CLABSI in tertiary gen-eral hospitals was higher than that in specialized hospitals(P<0.05).A total of 1268 strains of pathogenic bacte-ria were cultured and isolated,mainly Klebsiella pneumoniae,Staphylococcus epidermidis,Staphylococcus au-reus,Staphylococcus hominis,Acinetobacter baumannii and Pseudomonas aeruginosa.The distribution of patho-gens in secondary and tertiary hospitals was relatively consistent.Klebsiella pneumoniae,carbapenem-resistant Klebsiella pneumoniae(CRKP),Acinetobacter baumannii and carbapenem-resistant Acinetobacter baumannii(CRAB)were only sensitive to tigecycline.The E.coli had high resistance rates to cefazolin,cefuroxime,ceftri-axone,cefotaxime,levofloxacin,norfloxacin,moxifloxacin,ciprofloxacin and sulfamethoxazole/trimethoprim.The resistance rate of Pseudomonas aeruginosa to most antibiotics was lower than 30.00%.The resistance rates of Staphylococcus aureus and methicillin-resistant Staphylococcus aureus(MRSA)to erythromycin and clindamycin were higher than 53.39%.The resistance rates of Staphylococcus epidermidis and methicillin-resistant Staphylo-coccus epidermidis(MRSE)to erythromycin,clindamycin,levofloxacin,ciprofloxacin and sulfamethoxazole/tri-methopridine ranged from 47.33%to 73.08%.CONCLUSION There are some differences in the prevalence charac-teristics of CLABSI in different grades and categories of hospitals in this city,which should be combined with the local disease surveillance data and initial experience to effectively control the infection and slow down its spread.
