目的 探讨颅脑损伤合并肺部感染miRNA表达图谱及凝集素相关丝氨酸蛋白酶2(MASP-2)位点rs72550870突变的关联。方法 前瞻性纳入2019年3月—2020年8月邢台市人民医院急诊收治的95例颅脑损伤患者为研究对象,根据其术后是否发生肺部感染分为感染组23例和非感染组72例;收集两组患者临床资料,检测其外周血中单个核细胞miRNAs表达谱,MASP-2基因多态性及MASP-2基因位点rs72550870检测,并利用Logistic回归模型进行相关性分析。结果 两组患者入院时格拉斯哥昏迷(GCS)评分≥8分、术后卧床时间≥4周、术前血清白蛋白水平及外周血单个核细胞miRNAs表达水平对比,均存在统计学差异(P<0。05);改变倍数≥2倍的miRNA总数为42,其中24个表达下调、18个表达上调;感染组患者外周血miR-146a、miR-155、miR-125b及miR-22-3p水平均高于未感染组(P<0。05);miR-146a、miR-155、miR-125b及miR-22-3p水平检测的曲线下面积(AUC)分别为0。759、0。866、0。746、0。784(P<0。05);感染组MASP-2基因位点rs72550870基因型AA分布频率及等位基因A分布频率均较非感染组偏高(P<0。05);MASP-2基因位点rs72550870的等位基因A及基因型AA与颅脑损伤患者术后并发肺部感染相关(P<0。05)。结论 检测颅脑损伤患者的miRNA表达图谱可辅助临床评估其继发颅脑损伤的风险,其中MASP-2的位点rs72550870等位基因A和基因型AA突变是高危因素。
Association of expression profiles of miRNA in craniocerebral injury patients complicated with pulmonary infection with mutation at rs72550870 locus of MASP-2
OBJECTIVE To explore the association of expression profiles of miRNA in the craniocerebral injury pa-tients complicated with pulmonary infection with the mutation at rs72550870 locus of mannose-binding lectin-asso-ciated serine protease 2(MASP-2).METHODS A total of 95 patients with craniocerebral injury who were treated in emergency department of Xingtai City People's Hospital from Mar.2019 to Aug.2020 were prospectively re-cruited as the research subjects and were divided into the infection group with 23 cases and the non-infection group with 72 cases according to the status of postoperative pulmonary infection.The clinical data were collected from the two groups of patients,the expression profiles of miRNA in peripheral blood mononuclear cells were detected,the MASP-2 gene polymorphisms and rs72550870 locus of MASP-2 gene were detected,and the correlative analy-sis was performed by means of logistic regression model.RESULTS There were significant differences in Glasgow coma scale(GCS)score no less than 8 points at the admission to hospital,postoperative bedridden time no less than 4 weeks,preoperative serum albumin level and expression levels of miRNAs in peripheral blood mononuclear cells between the two groups(P<0.05).The total number of miRNA with the changed multiple no less than 2 was 42,24 of which were down-regulated expressions,and 18 were up-regulated expressions.The levels of pe-ripheral blood miR-146a,miR-155,miR-125b and miR-22-3p of the infection group were higher than those of the no infection group(P<0.05).The areas under curves(AUCs)of miR-146a,miR-155,miR-125b and miR-22-3p were 0.759,0.866,0.746 and 0.784,respectively(P<0.05).The distribution frequency of AA genotype at rs72550870 locus of MASP-2 gene and the distribution of A allele were higher in the infection group than in the non-infection group(P<0.05).The allele A and AA genotype at rs72550870 locus of MASP-2 gene were associat-ed with the postoperative pulmonary infection in the patients with craniocerebral injury(P<0.05).CONCLUSION The expression profiles of miRNA of the patients with craniocerebral injury may facilitate the clinical assessment of risk of secondary craniocerebral injury.The mutations of A allele and AA genotype at rs72550870 locus of MASP-2 gene are the high risk factors.
