EB病毒相关性传染性单核细胞增多症患儿并发肺炎的危险因素

目的 探讨人类疱疹病毒(EB)相关性传染性单核细胞增多症(IM)患儿并发肺炎的危险因素。方法 回顾性分析2020年3月—2023年3月苏州高新区人民医院收治的EB病毒致IM患儿187例的临床资料,根据是否并发肺炎分为肺炎组55例和非肺炎组132例;比较两组临床资料,分析EB病毒致IM患儿并发肺炎危险因素。结果 肺炎组血清C-反应蛋白(CRP)、降钙素原(PCT)、中性粒细胞/淋巴细胞(NLR)水平较非肺炎组更高(P<0。05);多因素Logistic回归分析结果显示,年龄、肝脏肿大、热程是EB病毒致IM患儿并发肺炎的危险因素(OR=2。109,2。079,1。972,P<0。05)。结论 EB病毒致IM患儿并发肺炎后血清CRP、PCT、NLR呈高表达;临床可根据危险因素给予患儿针对性预防及干预措施,以降低EB病毒致IM患儿并发肺炎的发生风险。
Risk factors for pneumonia in children with Epstein-Barr virus-associated infectious mononucleosis
OBJECTIVE To investigate the risk factors for pneumonia complication in children with human herpes-virus(EB)virus-induced infectious mononucleosis(IM).METHODS The clinical data of 187 children with Ep-stein-Barr viruse-induced IM admitted to Suzhou Hi-Tech District People's Hospital from Mar.2020 to Mar.2023 were retrospectively analyzed,and the children were divided into 55 cases of pneumonia group and 132 cases of non-pneumonia group according to whether they were complicated with pneumonia.The clinical data of the two groups were compared,and the risk factors for pneumonia in children with IM caused by Epstein-Barr virus.RESULTS The levels of C-reactive protein(CRP),procalcitonin(PCT)and neutrophil/lymphocyte(NLR)in pneumonia group were higher than those in non-pneumonia group(P<0.05).Multivariate Logistic regression a-nalysis showed that older age,hepatosplenomegaly and long duration of fever were risk factors for pneumonia complication in children with Epstein-Barr viruse-induced IM(OR=2.109,2.079,1.972,P<0.05).CONCLUSION Serum CRP,PCT and NLR were highly expressed in children with Epstein-Barr virus IM compli-cated with pneumonia.Clinical prevention and intervention measures could be given according to risk factors to re-duce the risk of pneumonia in children with Epstein-Barr viruse-induced IM.
