Risk factors for pulmonary infection in elderly patients with chronic heart failure and association with CYP27B1-1260 gene polymorphisms
OBJECTIVE To explore the risk factors for pulmonary infection in the elderly patients with chronic heart failure(CHF)and analyze the association with gene polymorphisms of cytochrome P450 family member 27B1(CYP27B1)-1260.METHODS A total of 305 elderly patients with CHF who were treated in Huaihe Hospital of Henan University from May 2020 to May 2023 were enrolled in the study and were divided into the infection group with 59 cases and the non-infection group with 246 cases according to the status of pulmonary infection.The clini-cal data were collected from the patients,logistic regression analysis was performed for the risk factors for the pulmonary infection in the elderly patients with CHF,and the CYP27B1-1260 gene polymorphisms were detected by means of polymerase-chain-reaction-restriction-fragment-length polymorphism(PCR-RFLP)assay.RESULTS Among the 305 elderly patients with CHF,59 had pulmonary infection,accounting for 19.34%.Diabetes melli-tus,chronic obstructive pulmonary disease,invasive operation and cardiac function raging between grade Ⅲ and grade Ⅳ were the risk factors for the pulmonary infection in the elderly patients with CHF(P<0.05).The fre-quencies of CC genotype and C allele at rs10877012 locus of CYP27B1-1260 gene were higher in the infection group than in the non-infection group,while the frequencies of AA genotype and A allele of the infection group were lower than those of the non-infection group(P<0.05).CONCLUSION The incidence of pulmonary infection in the elderly patients with CHF is associated with the diabetes mellitus,chronic obstructive pulmonary disease,cardiac function classification and invasive operation.The CYP27B1-1260 gene polymorphisms are associated with the pro-gression of illness condition of the elderly CHF patients with pulmonary infection.
