Risk factors for spontaneous bacterial peritonitis in liver cirrhosis patients and association with TRAF gene polymorphisms
OBJECTIVE To explore the risk factors for spontaneous bacterial peritonitis(SBP)in the liver cirrhosis patients and analyze the association with gene polymorphisms of tumor necrosis factor receptor-associated factor 6(TRAF6).METHODS Totally 153 patients with liver cirrhosis who were treated in Tongxiang First People's Hos-pital from Jun 2018 to Jan 2022 were enrolled in the study and were divided into the SBP group with 52 cases and the non-SBP group with 101 cases according to the status of complication with SBP.The TRAF6 gene polymor-phisms were analyzed.Univariate analysis and multivariate logistic regression analysis were performed for the risk factors for the SBP in the liver cirrhosis patients.RESULTS Of the 153 liver cirrhosis patients,52 were complicat-ed with SBP,accounting for 33.99%.The frequency of AA genotype at rs5030445 locus of TRAF6 gene was sig-nificantly higher in the SBP group than in the non-SBP group(P<0.05).The proportions of the patients with more than 60 years of age,grade C of Child-Pugh,complication of upper gastrointestinal hemorrhage,complica-tion of hepatic encephalopathy,complication of hepatorenal syndrome and the serum total bilirubin(TBil)level were significantly higher in the SBP group than in the non-SBP group;while the ascites albumin(Alb)level of the SBP group was significantly lower than that of the non-SBP group(P<0.05).The result of multivariate logistic a-nalysis showed that the more than 60 years of age,grade C of Child-Pugh,complication with hepatic encephalopa-thy and genotype AA at rs5030445 locus of TRAF6 were the risk factos for the SBP in the liver cirrhosis patients(P<0.05);the rise of ascites Alb level was the protective factor for SBP in the liver cirrhosis patients(P<0.05).CONCLUSION The incidence rate of SBP is 33.99%among the liver cirrhosis patients,the incidence is closely as-sociated with the TRAF6 gene polymorphisms.There are a variety of risk factors for the SBP in the liver cirrhosis patients.It can provide bases for formulating prevention measures.
