Association between TREM-1 gene polymorphism and complication of abdominal infection,prognosis of patients with severe acute pancreatitis
OBJECTIVE To explore the association between the triggering receptor expressed on myeloid cells 1(TREM-1)gene polymorphism and the complication of abdominal infection,prognosis of the patients with severe acute pancreatitis(SAP).METHODS A total of 158 patients with SAP who were treated in critical care medicine department of Zibo Central Hospital from Mar 2019 to Dec 2022 were recruited as the research subjects,78 of whom were complicated with abdominal infection and assigned as the infection group,and 80 were not complicated with infection and assigned as the no infection group.The patients of the infection group were divided into the sur-vival group with 60 cases and the death group with 18 cases according to 28-day prognosis.The expression level of serum TREM-1 was detected,the expression frequencies of the TREM-1 gene polymorphism were observed,and the multivariate analysis was performed for the risk factors for abdominal infection in the SAP patients.RESULTS Totally 115 strains of pathogens were isolated from the 78 SAP patients complicated with abdominal infection,70(60.87%)of which were gram-negative bacteria.The frequencies of AA genotype and A allele at rs2234246 locus were higher in the infection group than in the no infection group(P<0.05).Abdominal effusion,multiple organ dysfunction syndrome and AA genotype at rs2234246 locus of TREM-1 gene were the risk factors for the abdomi-nal infection in the SAP patients(P<0.05).The levels of interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)of genotypes rs2234246 and rs2234237 were higher in the infection group than in the no infection group(P<0.05).In the infection group and the non-infection group,the levels of IL-6 and TNF-α of the patients with genotype AA at locus of TREM-1 gene were higher than those of the patients with genotype GA and GG(P<0.05).There were no significant differences in the frequencies of genotypes and alleles at loci of TREM-1 gene between the survival group and the death group.CONCLUSION The abdominal effusion,multiple organ dysfunction syndrome and gen-otype AA at rs2234246 locus of TREM-1 gene are the risk factors for the abdominal infection in the SAP patients.Among the patients with genotypes rs2234246 and rs2234237,the levels of inflammatory indexes of the patients with infection are higher than those of the patients without infection,and the levels of inflammatory indexes of the patients with infection are higher than those of the patients without infection among the patients with genotype AA at locus of TREM-1 gene;there is no association between the locus of TREM-1 gene and the abdominal infection in the SAP patients.
