摘要

目的 分析不同病情程度盆腔炎患者磺基转移酶A1(SULT1A1)基因rs9282861位点多态性及不同病原体感染情况.方法 回顾性选取2019年1月-2022年8月新余市人民医院收治的116例盆腔炎患者纳入研究组,其中轻度、中度、重度分别为37、49、30例,另择同期健康女性125名作为对照组.分析研究组、对照组以及不同病情程度盆腔炎患者解脲支原体、人型支原体、淋球菌感染情况以及盆腔分泌物菌群分布,SULT1A1基因rs9282861位点多态性,分析解脲支原体、人型支原体、淋球菌感染、SULT1A1基因rs9282861位点多态性与盆腔分泌物菌群分布的相关性.结果 研究组SULT1A1基因rs9282861位点AA基因型频率高于对照组(P<0.05);研究组解脲支原体、人型支原体、淋球菌感染率,阴道菌群密度Ⅰ+Ⅳ、阴道菌群多样性Ⅰ+Ⅳ、微生态异常比例、病原菌总检出率高于对照组(P<0.05);轻度盆腔炎、中度盆腔炎、重度盆腔炎患者解脲支原体、人型支原体、淋球菌感染率,阴道菌群密度Ⅰ+Ⅳ、阴道菌群多样性Ⅰ+Ⅳ、微生态异常比例、病原菌总检出率呈升高趋势(P<0.05);Spearman相关性分析法结果显示,解脲支原体、人型支原体、淋球菌感染、SULT1A1基因rs9282861位点AA基因型与阴道菌群密度Ⅰ+Ⅳ、阴道菌群多样性Ⅰ+Ⅳ、微生态异常呈正相关(P<0.05).结论 SULT1A1基因rs9282861位点AA基因型盆腔炎易感性更高,解脲支原体、人型支原体、淋球菌感染为盆腔炎症的发生及发展的重要因素,且其与盆腔分泌物菌群分布密切相关.

Abstract

OBJECTIVE To analyze the rs9282861 polymorphism of sulfotransferase A1(SULT1A1)gene and the distribution of pathogens in patients with pelvic inflammatory disease.METHODS A total of 116 patients with pel-vic inflammatory disease admitted to Xinyu People's Hospital from Jan 2019 to Aug 2022 were retrospectively in-cluded in the study group,of which 37 were mild,49 were moderate and 30 were severe,respectively.One hun-dred and twenty-five healthy women during this period were recruited in the control group.The pathogenic distri-bution of ureaplasma urealyticum,mycoplasma hominis,gonococcal bacteria and pelvic secretions between the study group and control group,and among patients with different degrees of pelvic inflammatory dis-ease.Rs9282861 polymorphisms of SULT1A1 gene in the study group and control group were analyzed and the correlation with Ureaplasma urealyticum,Mycoplasma hominis,Gonorrhoeae infection and distribution of bacte-ria in pelvic secretions was analyzed.RESULTS The AA genotype frequency of rs9282861 site at SULT1A1 gene in the study group was higher than that in the control group(P<0.05).The infection rate of Ureaplasma urea-lyticum,Mycoplasma hominis and Gonococcus,the density of vaginal flora Ⅰ+Ⅳ,the diversity of vaginal flora Ⅰ+Ⅳ,the proportion of microecological abnormalities and the total detection rate of pathogenic bacteria in the study group were higher than those in the control group(P<0.05).The infection rates of Ureaplasma urealytic-um,Mycoplasma hominis and Gonococcus,the density of vaginal flora Ⅰ+Ⅳ,the diversity of vaginal flora Ⅰ+Ⅳ,the proportion of microecological abnormalities and the total detection rate of pathogenic bacteria in patients with mild pelvic inflammatory disease,moderate pelvic inflammatory disease and severe pelvic inflammatory dis-ease showed an increasing trend(P<0.05).Spearman correlation analysis showed that Ureaplasma urealyticum,Mycoplasma hominis,Gonococcus infection and rs9282861 AA genotype frequency of SULT1A1 gene were posi-tively correlated with vaginal flora density Ⅰ+Ⅳ,vaginal flora diversity Ⅰ+Ⅳ and proportion of microecological abnormalities(P<0.05).CONCLUSION SULT1A1 gene rs9282861 AA genotype has higher susceptibility to pel-vic inflammatory disease.Ureaplasma urealyticum,Mycoplasma hominis and Gonococcus infection were impor-tant factors for the occurrence and development of pelvic inflammatory disease,and were closely related to the dis-tribution of bacterial flora in pelvic secretions.
