Report on high-risk human papillomavirus infection and screening for cervical cancer and precancerous lesions among women in Hainan Province between 2019 and 2022
OBJECTIVE To analyze the status of high-risk human papillomavirus(HR-HPV)infection and its asso-ciation with cervical cancer and cervical precancerous lesions among women in Hainan Province.METHODS Data were obtained from the free cervical cancer screening programme for women in Hainan Province between 2019 and 2022.The specimens from all women were used for initial HPV screening and subsequent cytological examination,cytology,colposcopy and cervical histopathological examination were performed on those based on the results of the initial screening.RESULTS A total of 947 852 women in Hainan Province were enrolled in the study from 2019 to 2022,and the infection rate of HR-HPV was 10.91%,of which the infection rates of HPV 16,HPV 18,and other 12 types of HPV were 1.17%,0.70%,and 9.66%,respectively.The ethnic groups with the highest rates of HR-HPV infection were the Miao,other ethnic minorities and Zhuang,with the rates of 14.78%,11.43%,and 11.33%,respectively(P<0.05).The age groups presenting the highest HR-HPV infection rates were 20-,55-64 and 25-years old,with prevalence rates of 14.87%,12.45%,12.54%and 11.33%,respectively(P<0.05).Over-all HR-HPV infection rates of HPV 16,HPV 18 and other 12 types of HPV varied by city and county(P<0.05).The detection rate of cervical precancerous lesions in Hainan Province from 2019-2022 was 304.79/100 000,and the detection rate of cervical cancer was 15.40/100 000.Among HPV-positive individuals,the detection rates of cervical cancer were higher among those with HPV16,HPV16/18 and HPV18 infection(P<0.05).The detection rates of cervical cancer were higher among those aged 55-64 years and 45-years(P<0.05).CONCLUSION The distribution of HR-HPV infection among women of different ethnic groups,ages and regions in Hainan Province varies.The government departments should increase investment in further expanding primary HPV-based cervical cancer screening and incorporating HPV vaccination for low-age girls into free government programs,in order to eliminate and reduce regional disparities.
