CPIS评分及miR-127和miR-146a诊断老年重症肺炎患者预后的临床价值

目的 探讨老年重症肺炎(SP)患者感染病原菌分布及临床肺部感染评分(CPIS)、微小核糖核酸-127(miR-127)、微小核糖核酸-146a(miR-146a)对预后不良的诊断价值。方法 选取2021年1月-2023年3月在湖北医药学院附属国药东风总医院接受治疗的485例老年SP患者,将入院治疗1个月后死亡的45例患者纳入预后不良组,入院治疗1个月后生存的440例患者纳入预后良好组,比较两组临床资料,分析感染病原菌分布特点,采用受试者工作特征(ROC)曲线分析CPIS评分及miR-127和miR-146a对老年SP患者预后不良的诊断价值。结果 与预后良好组患者比较,预后不良组患者年龄较大(P<0。05),累及脏器数≥3个患者占比、机械通气次数、体内留置导管次数占比升高(P<0。05),CPIS评分升高(P<0。05);共检出病原菌659株,其中主要为铜绿假单胞菌和鲍氏不动杆菌;与预后良好组比较,预后不良组miR-127和miR-146a水平更高(P<0。05);CPIS评分及miR-127和miR-146联合诊断老年SP患者预后不良的临床价值更高(P<0。05),曲线下面积(AUC)为0。916,敏感度为82。22%,特异度为87。27%。结论 铜绿假单胞菌和鲍氏不动杆菌是老年SP患者感染的主要病原菌,CPIS评分及血清miR-127和miR-146a水平与患者预后密切相关,联合检测对患者预后具有更高的诊断价值。
Clinical values of CPIS score,miR-127 and miR-146a in diagnosis of prognosis of elderly patients with severe pneumonia
OBJECTIVE To explore the distribution of pathogens and analyze the values of clinical pulmonary infec-tion score(CPIS),micro ribonucleic acid-127(miR-127)and micro ribonucleic acid-146a(miR-146a)in diagnosis of poor prognosis of the elderly patients with severe pneumonia(SP).METHODS A total of 485 elderly patients with SP who were treated in Dongfeng General Hospital of Sinopharm Affiliated to Hubei University of Medicine from Jan 2021 to Mar 2023 were enrolled in the study,45 of whom died after the treatment for 1 month and were assigned as the poor prognosis group,and 440 survived after the treatment for 1 month and were assigned as the favorable prognosis group.The clinical data and distribution of pathogens causing infections were observed and compared between the two groups of patients.The values of the CPIS,miR-127 and miR-146a in diagnosis of the poor prognosis of the elderly patients with SP were analyzed by means of receiver operating characteristic(ROC)curve.RESULTS The patients of the poor prognosis group were significantly older than those of the favorable prognosis group(P<0.05).The proportion of patients with no less than 3 organs involved,number of times of mechanical ventilation and catheter indwelling were significantly increased(P<0.05);the CPIS was significantly increased(P<0.05).Totally 659 strains of pathogens were isolated,Pseudomonas aeruginosa and Acinetobacter baumannii were the predominant species of pathogens.The levels of miR-127 and miR-146a of the poor prognosis group were significantly higher than those of the favorable prognosis group(P<0.05).The area under curve(AUC)of the joint detection of CPIS,miR-127 and miR-146 was 0.916 in diagnosis of the poor prognosis of the elderly patients with SP,with the sensitivity 82.22%,the specificity 87.27%;the diagnostic value of the joint detection was significantly higher(P<0.05).CONCLUSION P.aeruginosa and A.baumannii are dominant a-mong the pathogens causing the infections in the elderly patients with SP.The CPIS,serum miR-127 and miR-146a are closely associated with the prognosis of the patients,and the joint detection of the above indexes has high value in diagnosis of the prognosis.
