Th17/Treg、sTREM-1及IGF-1预测老年2型糖尿病并发肺部感染的临床价值

摘要
目的 分析老年2型糖尿病(T2DM)合并肺部感染患者外周血辅助性T淋巴细胞17(Th17)/调节性T淋巴细胞(Treg)、可溶性髓样细胞触发受体-1(sTREM-1)和胰岛素样生长因子1(IGF-1)表达水平及其预测价值.方法 选取2021年1月-2023年4月锦州医科大学附属第一医院收治的老年T2DM合并肺部感染患者57例为感染组,61例未合并肺部感染患者为非感染组,检测外周血 Th17/Treg、sTREM-1和IGF-1表达水平,受试者工作特征(ROC)曲线分析各指标预测T2DM患者并发肺部感染的临床价值.结果 感染组Th17/Treg、sTREM-1表达水平高于非感染组(P＜0.05),IGF-1表达水平低于非感染组(P＜0.05);Th17/Treg、sTREM-1和IGF-1预测老年T2DM患者并发肺部感染的曲线下面积(AUC)值分别为0.764、0.784和0.779,三项联合AUC值为0.846.结论 老年T2DM患者合并肺部感染可引起Th17/Treg、sTREM-1和IGF-1水平异常表达,三者联合对老年T2DM患者并发肺部感染的预测价值更高.
Abstract
OBJECTIVE To investigate the expression levels of peripheral blood ratio of helper T lymphocytes 17(Th17)to regulatory T lymphocytes(Treg),soluble triggering receptor expressed on myeloid cells-1(sTREM-1)and insulin-like growth factor 1(IGF-1)in the elderly patients with type 2 diabetes mellitus(T2DM)and analyze the predictive values.METHODS A total of 57 elderly T2DM patients who were complicated with pulmonary in-fection and were treated in the First Affiliated Hospital of Jinzhou Medical University from Jan 2021 to Apr 2023 were assigned as the infection group,61 patients who were not complicated with pulmonary infection were chosen as the non-infection group.The expression levels of peripheral blood Th17/Treg,sTREM-1 and IGF-1 were de-tected,and the clinical values of the indexes in prediction of pulmonary infection in the T2DM patients were ana-lyzed by means of receiver operating characteristic(ROC)curves.RESULTS The expression levels of Th17/Treg and sTREM-1 of the infection group were significantly higher than those of the non-infection group(P＜0.05),the expression level of IGF-1 of the infection group was significantly lower than that of the non-infection group(P＜0.05).The areas under curves(AUCs)of Th17/Treg,sTREM-1 and IGF-1 were respectively 0.764,0.784 and 0.779 in prediction of pulmonary infection in the elderly patients with T2DM,the AUC of the joint de-tection of the three indexes was 0.846.CONCLUSION The pulmonary infection in the elderly T2DM patients may lead to the abnormal expressions of Th17/Treg,sTREM-1 and IGF-1.The joint detection of the three indexes has higher value in prediction of the pulmonary infection in the elderly patients with T2DM.
