Islet function and osteoporosis indexes of type 2 diabetes mellitus patients complicated with Helicobacter pylori infection and TNF-α gene polymorphisms
OBJECTIVE To explore the islet function and osteoporosis indexes of type 2 diabetes mellitus patients complicated with Helicobacter pylori infection and tumor necrosis factor-α(TNF-α)gene polymorphisms.METHODS A total of 163 patients with type 2 diabetes mellitus who were treated in Tangshan Workers Hospital from Jan 2022 to Mar 2023 were enrolled in the study and were divided into the infection group with 95 cases and the non-infection group with 68 cases according to the status of complication with H.pylori infection.The pan-creas islet function and osteoporosis-related indexes were observed and compared between the two groups of pa-tients,and the TNF-α gene polymorphisms were detected by means of polymerase-chain-reaction-restriction-fragment-length polymorphism(PCR-RFLP).RESULTS Among the 163 patients with type 2 diabetes mellitus,95 were complicated with H.pylori infection,with the infection rate 58.28%.The high-density lipoprotein cholesterol(HDL-C)of the infection group was significantly lower than that of the non-infection group(P<0.05);the levels of to-tal cholesterol(TC),triglyceride(TG)and low-density cholesterol(LDL-C)of the infection group were significantly higher than those of the non-infection group(P<0.05).The levels of pancreas islet function indexes homeostasis model assessment of β(HOMA-β)and homeostatic model assessment of insulin resistance(HOMA-IR)of the infection group were significantly higher than those of the non-infection group(P<0.05),while the fasting insulin(FINS)level of the infection group was significantly lower than that of the non-infection group(P<0.05).The levels of osteoporosis-related indexes osteocalcin(OC),special sequence of C-terminal telopeptide β of type Ⅰ collagen(β-CTX)and propeptide of type Ⅰprocollagen(PINP)of the infection group were significantly lower than those of the non-infection group(P<0.05).The frequencies of CC genotype and C allele at 863 locus of TNF-α gene were significantly lower in the infection group than in the non-infection group(P<0.05).CONCLUSION The incidence of H.pylori infection is high among the patients with type 2 diabetes mellitus.The H.pylori infection may result in the disorder of lipid metabolism,promote the insulin resistance and lead to the osteoporosis.The TNF-α gene polymorphisms may involve in the progression of illness condition of the type 2 diabetes mellitus patients with H.pylori infection.
