Risk factors for multidrug-resistant organisms infections in patients with perianal abscess and establishment of prediction model
OBJECTIVE To explore the risk factors for multidrug-resistant organisms(MDROs)infections in the patients with perianal abscess,establish the risk prediction model and analyze its predictive value.METHODS A total of 108 patients with perianal abscess who were treated in Xingtai People's Hospital from Jan 2020 to Dec 2022 were enrolled in the study and were divided into the MDRO infection group with 31 cases and the non-MDRO infection group with 77 cases according to the pathogens isolated from the patients with perianal ab-scess.Multivariate logistic regression analysis was performed for the risk factors for the MDROs infections,the risk prediction model was established,and the predictive value of the model was analyzed by means of receiver op-erating characteristic(ROC)curves.RESULTS Totally 112 strains of pathogens were isolated,108(96.43%)of which were gram-negative bacteria,and 4(3.57%)were gram-positive bacteria.Among the 112 strains of patho-gens,31(27.68%)were MDROs,including 6 strains of extended-spectrum β-lactamases(ESBLs)-producing Klebsiella pneumoniae,24 strains of Escherichia coli and 1 strain of methicillin-resistant Staphylococcus aureus(MRSA).Complication with diabetes mellitus,previous history of recurrent attacks and use of various types of antibiotics were the risk factors for the MDROs infections in the patients with perianal abscess(OR=1.732;1.970;2.347,P<0.05).The diagnostic value of the risk prediction model was high,the area under curve(AUC)was 0.874(95%CI:0.796-0.930)with the sensitivity 64.50%,the specificity 93.50%.CONCLUSION The pa-tients with gram-negative bacteria infection are dominant among the patients with perianal abscess,most of which were multidrug-resistant.The MDROs infection is associated with the complication with diabetes mellitus,previ-ous history of recurrent attacks and types of antibiotics.The established risk prediction model has high diagnostic value.
