Expressions of miR-21,NLRP3 and IL-1β in patients with diabetic foot infection and values in diagnosis of prognosis
OBJECTIVE To explore the expression levels of microRNA21(miR-21),nucleotide-binding oligomeriza-tion domain-like receptor protein 3(NLRP3)and interleukin-1β(IL-1β)in the patients with diabetic foot infection and analyze the values in assessment of prognosis.METHODS A total of 82 patients with diabetic foot infection who were treated in the North China Petroleum Administration General Hospital from Jan 2019 to Jan 2023 were assigned as the study group,meanwhile,61 diabetic foot patients who did not have any infection were chosen as the control group.The levels of miR-21,NLRP3 and IL-1β were detected and compared between the two groups and among the patients with different severity of infection.A 6-month follow-up was carried out for the patients with diabetic foot infection after the treatment,and the patients were divided into the favorable prognosis group and the poor prognosis group according to the treatment outcomes.The values of miR-21,NLRP3 and IL-1β in prediction of the prognosis were analyzed by means of receiver operating characteristic(ROC)curves.RESULTS The levels of NLRP3 and IL-1β of the study group were significantly higher than those of the control group,while the miR-21 level of the study group was lower significantly than that of the control group,and they changed with the aggravation of infection(P<0.05).There were significant differences in age,severity of infection,distribution of isolated pathogens,glycosylated hemoglobin(HbA1c),miR-21,NLRP3 and IL-1β between the favorable prog-nosis group and the poor prognosis group(P<0.05).ROC curve analysis showed that areas under curves(AUCs)of miR-21,NLRP3,IL-1β and the joint detection were respectively 0.865,0.646,0.809 and 0.940 in prediction of poor prognosis of the patients with diabetic foot infection.CONCLUSION The patients with diabetic foot infec-tion show the rise of NLRP3 and IL-1β and the decline of miR-21,and the joint detection of the three indexes has certain value in prediction of prognosis of the diabetic foot infection patients.
